摘  要:目的:研究肾上腺髓质素(AM)与细胞角质蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)联合检测对肺癌与肺结核鉴别诊断的意义。方法:应用放射免疫法(RIA)分别检测肺癌、肺结核组病人及对照组外周血中AM、CYFRA21-1、NSE、CEA含量。结果:①肺癌组病人AM、CYFRA21-1、NSE、CEA水平均明显高于肺结核组及对照组(P<0.01)。③四项联合检测可进一步提高敏感性和准确性至90.7%和92.2%,明显优于单项检测。结论:AM,CYFRA21-1,NSE,CEA的检测可作为肺癌诊断及与肺结核鉴别诊断的实验室指标,联合检测可进一步提高诊断敏感性和临床符合率。Objective: To explore the correlation between AM and pulmonary tumor and study diagnostic value of combined detection of AM,CYFRA21-1,NSE and CEA in differentiating lung cancer from pulmonary tuberculosis. Methods ; The levels of AM, CYFRA21-1, NSE and CEA in pulmonary tumor ,pulmonary tuberculosis and normal contral group were determined by radioimmunoassay(RIA) . Results :①The levels of AM ,CYFRA21-1,NSE,CEA in patients with lung cancer were higher than those in patients with pulmonary tuberculosis and normal contrals (P〈0. 01). ②The sensitibity and accuracy of the combined determination of four tumor markers were 90. 7% and 92.2% ,being higher than each single determination in diagnosis of lung cancer . Conclusion ;AM,CYFRA21-1, NSE,CEA can be regarded as markers of differentiating lung cancer from pulmonary tuberculosis and can significantly improve the sensitivity and the accuracy in diagnosis of lung cancer.
