首发精神分裂症患者探究性眼球轨迹运动研究

摘　　要：目的:研究首发精神分裂症患者探究性眼球轨迹运动,并与抑郁症、复发性精神分裂症患者作比较。方法:选择来源于2003-11/2004-12北京大学第六医院的住院和门诊患者。应用探究性眼球轨迹运动标记记录仪(Type-Ⅳ)对31例首发精神分裂症与31例抑郁症分别两次检查,两次检查间隔时间为(33.74±8.15)d。同时首发精神分裂症组在两次检查时以阳性和阴性量表(由阳性量表7项、阴性量表7项和一般精神病理量表16项,共30项,及3个补充项目评定攻击危险性组成)评定精神症状;抑郁症组以汉密顿抑郁量表(采用0～4分的5级评分法,0分为无此项症状,4分为极重度)评定抑郁症状。41例复发性精神分裂症进行一次探究性眼球轨迹运动检查,并与31例首发精神分裂症进行比较。结果:103例患者均进入结果分析,无脱落者。①眼球轨迹运动检查结果:眼球轨迹运动主要指标在首发精神分裂症组与复发性精神分裂症组比较其差别无显著性(P>0.05);首发精神分裂症组两次检查时眼球轨迹运动主要指标比较其差别无显著性(P>0.05);首发精神分裂症组第1次与第2次判别分析值高于抑郁症组、反应性探究分低于抑郁症组(1.82±1.76,0.52±1.58;1.81±1.51,0.79±1.44;8.10±1.89,9.39±1.63;8.26±1.69,9.13±1.65,P<0.05,P<0.01)。②阳性和阴性量表评定结果:首发精神分裂症组第2次检查时的阳性和阴性量表总分、阳性症状分、阴性症状分及综合分与第1次相比均降低(42.00±12.96,76.87±19.03;9.06±3.52,21.77±7.09;11.45±4.84,20.19±6.74;21.81±5.66,35.19±10.18,P<0.01)。③汉密顿抑郁量表评定结果:抑郁症组第2次检查时汉密顿抑郁量表评分与第1次相比降低(17.48±7.82,32.14±5.72,P<0.01)。结论:探究性眼球轨迹运动是精神分裂症患者的特征性指标;抗精神病性药物、病程长短对探究性眼球轨迹运动影响不大。精神分裂症和抑郁症比较其差异有显著性。探�AIM： To study eye tracking movements of first episode schizophrenia and explore the differences of eye tracking movements among first episode schizophrenia, recurrent schizophrenia and depression. METHODS： The subjects were selected from the patients of the Sixth Hospital of Peking University between November 2003 and December 2004. Thirty-nne first episode schizophrenics and 31 patients with depression received examinations by using eye tracking movement labeling recorder （Type-Ⅳ） twice with an interval of （33.74±8.15） days, at the same time positive and negative symptom schedule （PANSS） was applied to the first episode schizophrenics, and Hamilton rating scale for depression （HAMD） was applied to the patients with depression twice, 41 recurrent schizophrenics were tested by tracking of exploratory eye movement once and compared with the first episode schizophrenics. RESULTS： All the 103 cases were involved in the analysis if results. The results of eye tracking movements： no significant differences were noted in main indexes of eye tracking movements between first episode schizophrenics and recurrent schizophrenics（P 〉 0.05）,.no significant differences were noted in main indexes of eye tracking movements between the first and the second test of first episode schizophrenics （P 〉 0.05）, Diseriminant analysis （D） of the first and the second test of first episode schizophrenics are both higher than those of the depressiongroup , and responsive of search scores （RSS） of the first and the second test of first episode schizophrenics were both lower than those of the depression group （1.82±1.76, 0.52±1.58; 1.81±1.51, 0.79±1.44; 8.10±1.89, 9.39±1.63; 8.26±1.69, 9.13±1.65, P 〈 0.05, P 〈 0.01）.② The results of PANSS： the total points, the positive points, the negative points and general points of the second test in first episode schizophrenics are all lower than those of the first test （42.00±12.96, 76.87±19.03; 9.06±3.52, 21.77±7.09; 11.45±4.84, 20
