摘 要:目的:观察应用奥氮平和氟哌啶醇治疗精神分裂症患者血清催乳素和生长激素水平的影响。方法:选择2003-08/2004-10无锡市精神卫生中心精神科住院的精神分裂症患者61例,随机分为奥氮平组28例和氟哌啶醇组33例。两组均在第1周内加至常规治疗剂量,并连服8周,同期选取健康对照组30例。3组均采用放射免疫法测定血清催乳素和生长激素的水平,并采用直线回归法分析血清催乳素和生长激素水平与阳性和阴性症状量表评分的相关性。结果:按意向处理分析,61例患者均进入结果分析。①血清催乳素水平变化:治疗前奥氮平组与氟哌啶醇组水平接近(P>0.05)。治疗8周后奥氮平组较治疗前变化不明显(t=1.010,P>0.05),氟哌啶醇组显著高于治疗前犤(98.96±47.58),(16.58±10.38)μg/L,t=9.816,P<0.01犦。②生长激素水平变化:治疗前奥氮平组与氟哌啶醇组水平接近(P>0.05)。治疗8周后奥氮平组较治疗前变化不明显(t=0.2078,P>0.05)。氟哌啶醇组显著低于治疗前犤(1.57±1.68),(2.4±1.98)μg/L,P<0.01犦。③血清催乳素和生长激素水平与阳性和阴性症状量表评分的相关性:治疗前奥氮平和氟哌啶醇组患者阳性和阴性症状量表评分与血清催乳素和生长激素无明显相关性(r=0.081,-0.283;P>0.05)。治疗后奥氮平组阳性和阴性症状量表评分与血清催乳素和生长激素相关系数为r=-0.162,-0.263,氟哌啶醇组为r=0.238,-0.173;P均>0.05,无明显相关性。结论:应用奥氮平治疗对患者血清催乳素和生长激素的影响不明显,临床治疗中选用非典型抗精神病药奥氮平可以降低药物的风险和减少药物的副反应。AIM: To find out if there was any difference of serum prolactin (PRL) and growth hormone (GH) level between schizophrenics and normal control, to observe the change of these two hormone after treatment of antipsychotics, and compare the hormone change after Olanzapine and Haloperdol treatment. METHODS; R1A was used to examine serum levels of PRL and GH in 61 schizophrenics before treatment, after 8 weeks' treatments; the 61 patients were randomly assigned to Olanzapine group (28 cases) and Haloperdol group (33 cases), the results before treatment were compared with those of 30 normal control. RESULTS: There was no difference of serum PRL level and GH level between schizophrenics before treatment and normal control (P 〉 0.05). After treatment of antipsychotics, in the Olanzapine group, there were no differences about PRL level and GH level before and After treatment;in the Haloperdol group, after 8 weeks' treatment, the prolactiin level of patients was (98.96±47.58)μg/L significantly higher than (16.58±10,38)μg/L before treatment (P 〈 0.01), the serum GH level of patients was (1.6±1.4)μg/L significantly lower than (2.4±1.6)μg/L before treatment (P 〈 0.05). There was no a significantly relationship between PRL and GH levels and the scores of PANSS in the patients after treatment with olanzapine and haloperdol. CONCLUSION: There are no difference of serum PRL level and GH level between schizophrenics before treatment and normal control. There is no significantly effect of Olanzapine on serum levels of PRL and GH. There is significantly effect of haloperidnl on serum levels of PRL and GH.
