摘　　要：目的深入探讨原发性肝细胞肝癌(HCC)病人外周血谷氨酰转肽酶(GGT)mRNA-H亚型表达与肝癌术后预后指标的关系,并与AFP比较,寻找评估肝癌预后更可靠的新指标。方法对78例HCC病人外周血应用逆转录聚合酶链反应方法检测GGTmRNA-H亚型;并运用放射免疫测定法检测AFP水平。对49例无门脉和(或)肝外转移的HCC病人进行随访研究。结果有肝内转移、门静脉癌栓及远处转移组其外周血GGTmRNA-H亚型阳性率显著高于对照组(P<0·05);在随访期内外周血GGTmRNA-H阳性组门脉及肝外转移的发生率比例明显高于阴性组(P<0·05),而AFP阳性及阴性组的转移发生率相似。结论HCC病人外周血细胞GGTmRNA-H亚型表达阳性者,术后出现转移的可能性高、预后差;GGTmRNA-H亚型与AFP相比,有望成为评估肝癌预后更可靠的新指标。Objective To compare the peripheral blood γ-glutamyl trasnpeptidase （GGT） mRNA subtypes and AFP for prognosis in patients with HCC after operation. Methods The GGT mRNA-H subtypes were determined by RT PCR and AFP by RIA in samples of peripheral blood from 78 patients with HCC. Meanwhile, an 18-month follow up was conducted for the 49 patients without portal vein and/or extrahepatic metastasis. Results The positive rates of GGT mRNA-H subtypes in peripheral blood were significantly higher in the non intrahepatic metastasis group, non portal vein thrombosis group and non-distant metastasis group than in the control （P〈0.05）. The follow-up showed that the rate of portal vein and/or extrahepatic metastasis was markedly higher in the GGT mRNA-H （＋） patients than in the GGT mRNA-H （-） ones （P〈0.05）. However, there was no significant difference in the rate between AFP（ ＋ ） group and AFP（-） group （P〉0.05）. Conclusions The level of GGT mRNA-H subtypes in peripheral blood can not only reflect the metastasis of tumor in patients with HCC but also show the prognosis. Therefore, it can be used as a novel prognostic maker for HCC.
