胰腺癌组织中C-erbB-2、P53及PCNA的表达及临床意义

摘　　要：目的:研究胰腺癌中C-erbB-2、P53和增殖细胞核抗原(PCNA)的表达及其与临床病理学特征的关系,为胰腺癌的诊断及治疗提供理论依据。方法:39例胰腺癌组织取自手术标本,并经病理证实。应用免疫组织化学法检测C-erbB-2、P53及PCNA的表达。结果:胰腺癌组织中C-erbB-2、P53的阳性表达率分别为51.3%和59.0%,PCNA低、高增殖活性表达率分别为41.0%和59.0%。C-erbB-2阳性表达率与淋巴结转移、局部浸润、分化程度及临床分期等均显著相关(均为P<0.05)。P53表达率与临床分期、组织分化程度显著相关(均为P<0.05),而与淋巴结转移及局部浸润无关(均为P>0.05)。伴有淋巴结转移的癌组织中PCNA高增殖活性表达率明显增高(P<0.05),在低分化与高中分化胰腺癌组织中PCNA增殖指数具有明显差异(P<0.05)。C-erbB-2阳性率与肿瘤增殖活性相关(r=0.543,P<0.01),而与P53的表达无关。结论:C-erbB-2、P53及PCNA与胰腺癌发生、发展关系密切,是反映胰腺癌生物学行为的重要指标。Objective To investigate the expression of C-erbB-2, P53 and proliferating cell nuclear antigen（PCNA） and their relationship with clinical pathological characteristics in human pancreatic adenocarcinoma aiming at providing theoretical basis for clinical diagnosis and treatment. Methods C-erbB-2, P53 and PCNA in 39 cases of pancreatic adenocarcinoma taken from postoperative specimen were detected：using lmmunohlstochennstry of avidin-biotin complex method. Results The positive rate of C-erbB-2 and P53 were 51.3% and 59.0%, whereas thepositive rate of lower and higher proliferating activity of PCNA were 41.0% and 59.0% respectively. The expression rate.of C-erbB-2 was positively correlated with lymph node metastasis, local infiltration, differentiation degree and clinical stage（ P〈 0. 05）. The positive rate of P53 was associated with clinical stage and differentiation degree, but not related with lymph node metastasis and local infiltration. Proliferating activity of PCNA was higher in cases with lymph node metastasis than that without lymph node metastasis, and labeling index of PCNA was significantly different between high and low differentiation. The expression rate of C-erbB-2 was correlated with proliferating activity of tumor, but no corelation was found between the expressions of C-erbB-2 and P53. Conclusion C-erbB-2, P53 and PcNA are closely involved in the genesis and development of pancreatic carcinoma, and they are important markers reflecting pancreatic carcinoma behavior.
