摘 要:目的探讨脑室内脑膜瘤的临床特点及显微手术方法,提高手术治疗效果。方法13例脑室内脑膜瘤(侧脑室脑膜瘤11例,第四脑室脑膜瘤2例),术前行颅脑CT、MRI和脑血管造影检查,术中采用显微技术及美国产Nd:YAG激光等辅助设备。结果肿瘤全切除12例,次全切除1例。无手术死亡,无严重或永久性并发症发生。结论脑室内脑膜瘤通过影像学检查可确诊,显微手术是肿瘤全切除、降低病死率及致残率的关键,应用激光辅助治疗有利于控制出血和肿瘤全切除。Objective To study the procedure of microsurgery and clinical features of intraventricular meningioma for improving the therapeutic efficacy. Methods Thirteen cases with intraventricular meningioma (lateral ventricular meningioma 11 cases, fourth ventricular meningioma 2 cases) were investigated on CT scan, magnetic resonance imaging (MRI) and cerebral vascular angiography before operation. All of them were operated upon using microsurgical technique and Nd: Y AG laser. Results The tumor was removed totally in 12 cases, subtotaUy in 1 case. There was no surgical mortality. No severe and permanent complications occurred postoperatively. Conclusions Intraventricular meningioma can be diagnosed by CT scan or MRI, and the blood supply of the tumor can be revealed by vascular angiography. Microsurgery is the first choice in treatmeat of intraventricular meningioma, which is the key to increasing the total resection rate and reducing the mortality and invalidity rate. It is helpful in reducing bleeding and total removal of the tumor.
