摘 要:①目的探讨降钙素(CT)基因的高度甲基化与肿瘤发生、发展的关系及其对肿瘤诊断、鉴别诊断及预后判断的价值.②方法从肿瘤组织和癌旁组织中提取微量DNA,采用限制性内切酶HapⅡ和多聚酶链反应技术,测定了103例肿瘤病人肿瘤组织和其中30例病人的癌旁组织CT基因的甲基化程度.③结果 30例癌旁组织无1例检测到CT基因高度甲基化,而77.50%(31/40)的结直肠癌、56.25%的(27/48)肺癌和26.67%(4/15)的甲状腺腺瘤病人检测出CT基因高度甲基化,与癌旁组织比较,差异均有显著性(χ2=5.796~46.015,P<0.05、0.01);不同类型的肺癌病人,CT基因高度甲基化的阳性率不同,小细胞肺癌病人的阳性率明显高于鳞状细胞癌、腺癌和大细胞癌病人(P=0.030、0.010、0.011).随访4例高度甲基化阳性的甲状腺腺瘤病人,有2例分别在之后的8、14个月发生了恶变,而11例CT基因高度甲基化阴性的甲状腺腺瘤病人同期内无1例发生恶变.④结论CT基因的高度甲基化与肿瘤的发生、发展密切相关,可作为恶性肿瘤恶性克隆增殖的标志,为肿瘤的诊断、鉴别诊断及预后判断开辟了新的途径.Objective To study the relationship between the hypermethylation of ealeitonin(CT) gene and the genesis and development of tumors, and its value in the diagnosis, differential diagnosis and the prediction of prognosis of them. Methods Restricted digestion of DNA with Hpa Ⅱand polymerase chain reaction(PCR) were used to examine the methylation patterns of the CT gene in the primary lesions of 103 cases of tumors, in which 30 adjacent normal tissues of them included. Results No hypermethylation were detected in the adjacent normal tissues. The hypermethylation of CT gene was found in 31 of 40 cases of the colon or rectal carcinoma (77.50%), 27 of 48 lung cancers (56.25%) and 4 of 15 thyroid adenomas (26.67%). The difference between the two groups was significant (χ^2 = 5. 796-46. 015;P〈0.01, 0.05). There was an obvious difference between small-cell carcinoma and other three types of lung cancer (P=0. 030, 0. 010,0. 011). A follow-up study of four hypermethylated thyroid adenomas showed that two cases had canceration 8,14 months after the detection of hypermethylation of CT gene, while no canceration was noted in the 11 negative cases. Conclusion There was a close relationship between the hypermethylation of CT gene and the genesis and development of tumors. CT gene hypermethylation may be used as a malignant proliferative marker to open up a new way for the diagnosis, differential diagnosis as well as the prediction of prognosis of tumors.
