自拟肠粘连通畅汤治疗粘连性肠梗阻临床分析

摘　　要：目的:探讨自拟肠粘连通畅汤治疗粘连性肠梗阻的临床疗效。方法:将20例患者随机分为两组。对照组8例,给予禁食、持续胃肠减压、补充水电解质液、维持酸碱平衡等一般治疗。治疗组12例在应用以上方法的基础上,采用自拟肠粘连通畅汤(大黄、厚朴、枳实、炒莱菔子、芒硝、桃仁、赤芍、生白术、大腹皮、皂角刺)给予口服或由胃管注入。结果:治疗组:有效10例,其治疗时间小于24h者6例,24h^48h者3例,48h以上者1例,第1次排便时间平均为(16·9±4·7)h;中转手术者2例。对照组:有效5例,其治疗时间小于24h者2例,24~48h者3例,第1次排便时间平均为(34·3±5·3)h;中转手术者3例。两组比较有显著性差异(P<0·05)。结论:自拟肠粘连通畅汤治疗粘连性肠梗阻是一种安全有效的治疗方法。Objective：To explore the clinical effects of the apply user-defined intestines adhesive soup to treatment adhesive ileums. Methocls：20 cases were randomly divided into the control group of 8 cases and the therapy group of 12 cases. In the control group, treatment like fasting, continuous gastro-intestinal decompression, supplementing electrolytic solution, maintaining acid base balance was given to the patients. In the therapy group, apply above methods, in addition to user-defined intestines adhesive soup（ rhubarb, magnolia bark. immature bitter orange.stir-baked radish seed.mirabilite, red peony root. unprepared white atraotylodes rhizome, pericarpium precae, spina gleditsiae）oral administration or gastric canal infusion were given to the patients. Results ： In the treatment group, the treatment time was less than 24 hours in 6 cases, about 24-48 hours in 3 cases and more than 48 hours in 1 case. The average time of the first defecation occurred 16.9 ± 4.7 hours later. 2 cases received evacuation treatment. In the control group, the treatmen＇t time was less than 24 hours in 5 cases, 24 - 48 hours in 2 cases. The average time of the first defecation was （34.3 ± 5.3） hours. 3 cases received evacuation treatment. There existed marked difference between the two groups（P 〈 0.05）. Conclusion ：User-defined intestines adhesive soup is an effective method in the treatment of adhesive ileums.
