摘  要:目的分析短暂性脑缺血发作(TIA)的临床表现与MR脑灌注成像(PWI)和DSA检查结果之间的关系,探讨其临床应用价值。资料与方法30例颈内动脉系统TIA患者根据临床表现分为伴有肢体症状组(15例)和不伴有肢体症状组(15例)。在发作的间歇期分别行PWI、DSA检查,对临床表现与PWI及DSA的结果作对比分析。结果PWI检查结果显示,伴有肢体症状组均发现灌注异常区,不伴有肢体症状组只7例发现灌注异常区;经χ2检验,有无肢体症状与灌注异常有明显相关性(P<0.01)。DSA检查显示,伴有肢体症状组3例颈内动脉重度狭窄,12例颈内动脉闭塞;不伴有肢体症状组9例颈内动脉重度狭窄,6例颈内动脉闭塞;经χ2检验,有无肢体症状与血管狭窄程度无明显相关性(P>0.01)。结论PWI和DSA检查能够为TIA患者临床诊断提供有价值的客观影像学依据。Objective To study the clinical values of MR perfusion-weighed imaging (PWI) and digital subtraction angiography (DSA) in patients with transient ischemic attacks (TIA), and also discuss their correlations with clinical manifestations. Materials and Methods 30 patients with TIA were divided into 2 groups: group 1 with limb weakness or numbness, another group without limb weakness or numbness. PWI and DSA were performed on 30 patients with TIA. The relationships of clinic manifestation and the results of PWI and DSA were analyzed. Results Persisting abnormal perfusion areas were found in all cases of group 1 on PWI. Abnormal perfusion areas were found in 7 cases of group 2. There was statistically significant difference between clinical symptoms and abnormal perfusion( P 〈 0.01 ). Internal carotid artery stenosis was found in 3 cases and occlusion in 12 cases of group 1 on DSA. Internal carotid 'artery stenosis was found in 9 cases and occlusion in 6 eases of group 1 on DSA. There was no statistically significant difference between clinical symptoms and degree of internal carotid artery stenosis (P 〉 0.01 ). Conclusion PWI and DSA could provide valuable diagnostic information for patients with TIA.
