经尿道联合应用汽化与切割治疗中晚期前列腺癌

摘　　要：目的评价经尿道前列腺汽化电切除并睾丸去势术联合药物治疗中晚期前列腺癌的疗效。方法对24例中晚期前列腺癌患者行经尿道前列腺汽化加电切术并同期切除双侧睾丸,术后口服缓退瘤或磷酸雌二醇氮芥,随访1￣7a。结果手术前后对比,国际前列腺症状评分(IPSS)由(23.2±5.2)降至(9.2±4.6),最大尿流率(Qmax)由(5.6±2.0)mL/s升至(15.6±3.0)mL/s,血清PSA由53.6μg/L于术后3个月降至3.5μg/L。11例转移性骨痛患者术后症状部分缓解或消失,手术总时间为30￣120min,平均62.5min。结论该方法是治疗中晚期前列腺癌的安全有效手段,可明显减轻梗阻症状且提高生存质量。[Objective] To evaluate the effect of transurethral prostatectomy and castration on advanced prostate cancer. [Methods] 24 eases of advanced prostate cancer were treated by pharmoeotherapy with Flutamide or Extramustine phosphate after transurethral eleetrovaporization and resection of prostate and castration. [Results] After 1 month of operation, IPSS dropped from （23.2±5.2） to （9.2±4.6）, the maximum flow rate elevated from （5.6±2.0） mL/s to （15.6±3.0） mL/s. Serum PSA decreased from 53.6 ug/L to 3.5 ug/L 3 months after Flutamide therapy. The response of bone pain in 11 cases were 100%. [Conclusions] The method is safe and effective to treat advanced prostate cancer. It is helpful to improve the life quality of the patients by releasing the obstruction.
