苓苯二甲酸正丁酯净化自体骨髓移植治疗白血病

摘　　要：邻苯二甲酸正丁酯（ＤＢＰ）在体外具有选择性杀伤白血病细胞的特性，白血病骨髓体外培养体系中加入ＤＢＰ会加速白血病祖细胞的清除。在骨髓体外长期培养基础上，利用ＤＢＰ对１４例急性非淋巴细胞白血病病人骨髓进行体外净化并自体骨髓移植，骨髓细胞经过５～９ｄ与５０μｇ／ｍｌＤＢＰ共培养后，骨髓有核细胞回收率为６８％，ＣＦＵ－ＧＭ回收率达７０．１％。回输骨髓细胞悬液中检测不出残留ＤＢＰ。净化后骨髓回输给放、化疗预处理后病人，骨髓造血全部重建。至今１４例病人平均存活时间已达１５个月，２例复发，３例非白血病死亡，其余均无病存活。初步证明ＤＢＰ结合骨髓体外培养是一种安全有效的体外净化措施。It was proved that dibutyl phthalate DBP possesses the peculiarities in selectively killing leukemic cells or accelerating the deterioration of residual leukemic cells in long rerm marrow culture in vitro . Based on this principle, the DBP purged autologous bone marrow transplantation was performed in a group of 14 patients suffering from acute non lymphocytic leukemia. After 5￣9 days of in vitro co culture of marrow cells with DBP at the concentration of 50 μg/ml, the recovery of total nucleated cells and the number of CFU GM were 68% and 70.1% respectively, the residual DBP remaining in collected cells was below the detectable level. In all patients, the reconstitution of hematopoiesis after pre conditioning and transfusion of purged marrow cells was observed. Among them, 2 patients relapsed, 3 patients died of non leukemia disease, the others were alive free disease, the median duration of survival was 15 months.These preliminary clinical data support that marrow culture in the presence of DBP is a safe and effective measure in purged autologous bone marrow transplantation in the treatment of leukemia.
