眉脂肪垫瓣的解剖学基础研究

摘　　要：目的:为眉脂肪垫瓣修复上睑畸形的新术式提供解剖学基础。方法:15例成人标本解剖观察眉脂肪垫的位置、界限、面积大小、厚度、毗邻关系及血管构筑,模拟设计并形成眉脂肪垫瓣,测量瓣的大小、蒂部的位置、瓣翻转后所能覆盖上睑的范围。结果:眉脂肪垫位于眶上神经血管束及皱眉肌的外侧,眉外侧2/3,在额肌、眼轮匝肌眶部与骨膜之间,长度为(3.9±0.1)cm,在眉中心垂直线上宽度为(1.9±0.2)cm,厚度为(1.8±0.6)mm。其血供主要来自眶上动脉的分支,眶上神经是眉脂肪垫内侧的重要毗邻结构。结论:眉脂肪垫瓣的大小可设计成约4.0cm×2.0cm,根据眉脂肪垫动静脉进入眉脂肪垫的位置,眉脂肪垫瓣的蒂部设计在经瞳孔垂直线与眉毛相交处,宽度约1cm,瓣翻转后足以覆盖上睑,可修复上睑凹陷粘连等畸形。Objective： To establish the anatomical basis for using brow fat pad flap to repair the upper eyelid deformity. Methoda： The location, scope, dimension, thickness, surrounding area and blood vessels architecture of the brow fat pad were investigated by means of routine dissection in 15 adult cadaveric specimens. The brow fat pad flap in the cadaveric specimens were created under investigation. Its dimension, the location of the pedicle of the flap and the scope of the upper eyelid covered by the flap after turning over were determined. Results： The brow fat pad lied in the lateral to the supraorbital nerve and the corrugator,and was deep to the lower frontalis muscles and superficial to the periosteum. The average length, width and thickness was （3.9±0.1）cm, （1.9±0.2）cm and （1.8±0.6）mm respectively. Its blood supply came from the branch of the supraorbital artery. The branch of supraorbital nerve was important to the surrounding structure of the brow fat pad. Conclusions： The brow fat pad flap can be designed as 4.0 cm×2.0 cm. The pedicle of the flap can be designed at the place where the vertical line via pupil intersects the brow, and its width is about 1.0cm. The brow fat pad can be developed into brow fat pad flap, after turning over, to repair the sunken upper eyelid deformity.
