摘 要:目的探讨免疫球蛋白重链(IgH)及T细胞受体γ(TCRγ)基因重排在非霍奇金淋巴瘤(nonHodgkin’slymphoma,NHL)临床诊断中的应用。方法用聚合酶链反应(PCR)技术对58例B细胞淋巴瘤(BNHL)、15例T细胞淋巴瘤(TNHL)、不能确诊为NHL的标本5例及10例良性淋巴组织增生标本进行IgH及TCRγ基因重排的检测。结果58例B细胞淋巴瘤标本中有IgH基因重排为47例,有3例TCRγ基因重排。15例T细胞淋巴瘤中TCRγ基因重排为11例,未见有IgH基因重排。5例未确诊的疑难病例均为IgH基因重排。10例良性淋巴组织反应性增生病例中,IgH及TCRγ基因重排均为阴性。IgH及TCRγ基因重排检出率在淋巴瘤组与良性组之间,差异有显著意义(P<0.05)。结论用PCR方法检测IgH及TCRγ基因重排对NHL及其疑难病例有重要的诊断价值。Objective To assess the significance of detecting the gene rearrangements of IgH and TCRγ in diagnosis of non-Hodgkins lymphoma. Methods To detect the IgH and TCRγ gene rearrangements in 58 cases of B-NHL,15 cases of T-NHL,5 cases of suspected lymphomas and 10 cases of reactive lymphoid hyperplasia by using polymelase chain reaction. Results IgH gene rearrangements were found in 47 cases out of 58 cases of B cell lymphomas. Gene rearrangements of TCRγ were found in 3 out of 58 cases of B-NHL. Gene rearrangements of TCRγ were found in 11 cases out of 15 cases of T-NHL. IgH gene rearrangements were not found in T cell lymphomas. There were IgH gene rearrangements in 5 cases of suspected lymphomas. TCRγ gene rearrangements were not found in 5 cases of suspected lymphomas. Gene rearrangements of IgH and TCRγ were negative in 10 cases of reactive lymphoid hyperplasia. There was remarkable difference between NHL group and benign group of gene rearrangment (P 〈 0.05). Conclusion To detect the gene rearrangements of IgH and TCR7 by using PCR method can be useful in adjuvant diagnosis of non-Hodgkins lymphoma.
