电化学治疗晚期食管癌、贲门癌100例临床分析

摘　　要：１９９３年７月至１９９５年７月应用电化学治疗中、晚期食管癌、贲门癌１００例，其中８７例食管癌，１３例贲门癌。Ｘ线造影病变位于胸上段１５例，中段４５例，下段２７例，贲门１３例，肿瘤侵犯食管长度３．０～５．０ｃｍ２５例，５．１～７．０ｃｍ５０例，７．１～９．０ｃｍ１９例，＞９ｃｍ６例。８７例食管癌内，髓质型４７例，缩窄型２９例，覃伞型８例，溃疡型３例，另１３例贲门癌未分型。病理诊断：鳞癌７８例，腺癌２２例。本组无Ⅰ期（Ｔ１Ｎ０Ｍ０）病例，Ⅱ期（Ｔ２Ｎ０Ｍ０）２５例，Ⅲ期（Ｔ３Ｎ１Ｍ０）６９例，Ⅳ期（Ｔ１Ｎ２Ｍ１）６例，全组病例均有进食梗阻症状。根据ＵＩＣＣ标准评定，１００例经６～２４个月观察，其近期疗效ＣＲ１５例，ＰＲ５９例，ＮＣ１５例，ＰＤ１０例，其中ＣＲ＋ＰＲ共７５例占７５％。远期随访５７例，生存１３个月至２４个月。From July 1993 to July 1995, 100 cases of advanced esophageal or cardiac carcinonma were treated with eleatrochemical therapy. Among them, 87 cases were esophageal carcinoma; 13 cases were cardiac carcinoma. Radiographic examination showed that the case's number of lesions located in the upper, middle esophagus and in the cardiac was 15, 45, 27 and 13 respectively. The length of lesions was3.0 to 5.0 cm in 25 cases, 5.1 to 7.0cm in 50 cases, 7.1 to 9.0cm in 19 cases, over 9cm in 6 cases. Among 87 cases with esophageal carcinoma, medullary type, constrictive type. umbrella type and ulcerative type were in 47, 29, 8.3 cases respectirely. The 13 cases with cardiac carcinoma were not classified. In this group, 78 cases were squanous carcinoma and 22 cases adernocarcinoma. 25 cases were in stage Ⅰ(T_1N_0M_0) or Ⅱ(T_2N_0M_0), 69 cases in stage Ⅲ(T_3N_1M_0), 6 cases in stage Ⅳ period(T_1N_2M_1). All cases had symptom of dysphagia. According to UICC criteria, 15 cases obtained completed remission(CR), 59 cases obtained partial remission(PR). The total response rate was 75%. In 57 cases followed up for 24 monthsm, the survival time was 13 to 24 months.
