注意缺陷障碍伴多动与铅、镁及智商的关系

摘　　要：目的:探讨注意缺陷障碍伴多动与铅、镁及智商的关系。方法:选择2003-01/2004-06就诊于泰山医学院附属医院门诊的6～12岁注意缺陷障碍伴多动患儿65例为病例组,并选择按年龄、性别1∶1匹配的正常儿童65例。用Conners儿童行为问卷和韦克斯勒学龄儿童智力量表测定儿童的行为及智力。用原子吸收分光光度计石墨炉法、火焰法测定血铅、血镁。测定发铅、发镁。结果:所纳入的65例患儿及65例健康儿童均进入结果分析,无脱落者。①两组儿童的元素含量及智商比较:注意缺陷障碍伴多动组血铅高于对照组[(220.38±25.43),(121.17±19.20)μg/L],血镁、发镁低于对照组[(28.35±3.67),(42.15±3.99)mg/L,(14.54±4.39),(22.50±4.78)μg/g],总智商、言语智商、操作智商低于对照组(100±12,111±13;19±3,22±5;20±4,24±4)。②血、发元素含量、智商及行为量Pearson相关分析:血铅与行为量表得分呈显著正相关(r=0.63,P﹤0.001),血镁、总智商、言语智商、操作智商与行为量表得分呈不同程度的负相关(r=-0.32～0.46,P﹤0.05～0.001)。③注意缺陷障碍伴多动影响因素的Logistic回归分析:Logistic逐步回归分析显示血铅为注意缺陷障碍伴多动发病的危险因素,血镁与智商为注意缺陷障碍伴多动发病的保护因素。结论:注意缺陷障碍伴多动与血铅、镁有关,降低体内铅含量、适当补镁或防止低镁可能有助于注意缺陷障碍伴多动的防治。AIM：To explore the relationship among attention deficit hyperactivity disorder （ADHD）, Pb, Mg and intelligence quotient. METHODS：Sixty-five children with ADHD aged from 6-12 years old were selected as case group from out-patient clinic of Affdiated Hospital of Taishan Medical College between January 2003 and June 2004. Sixty-five healthy children were selected according to the proporlion of one to one in age and sex.The behavior and intelligence quotient were detected with Conners children behavior questionnaire and Wechsler Intelligence Scale for Children （WISC）.The Ph and Mg in plasma were measured with atomic absorption spectrophotometer （Graphite stove and blaze method）. Pb and Mg in hair were measured, too. RESULTS： The included 65 children patients and 65 healthy children were all involved in the result analysis without drop. ① Element content and intelligence quotient in children in the two groups：The Pb in plasm in the ADHD group was higher than that in the control group [（220.38±25.43）, （121.17±19.20）μg/L],and the Mg in plasm and hair was lower than that in the control group [（28.35±3.67）, （42.15±3.99） mg/L, （ 14.54 ± 4.39）, （22.50 ± 4.78）μg/g].The total intelligence quotient, language intelligence quotient and operation intelligence quotient were lower than those in the control group （100±12,111±13;19±3,22±5; 20±4,24±4）. ② Element content in plasm and hair, intelligence quotient and behavior Pearson correlation analysis：The Pb in plasm had significant positive correlation with behavior scale （r=0.63,P〈 0.001）, and the Mg in plasm, total intelligence quotient, language intelligence quotient and operation intelligence quotient had negative correlation in different degree with behavior scale （r=-0.32-0.46,P〈 0.05-0.001）. ③ Logistic regression analysis of ADHD influence factors：The Logistic step regression analysis indicated that the Pb in plasm was a dangerous factor of illness due to ADHD,while blood Mg and intelligence qu
