外伤眼穿透性角膜移植玻璃体视网膜联合手术

摘　　要：目的评价眼外伤患者穿透性角膜移植玻璃体视网膜联合手术的疗效.方法回顾性分析自2002年3月～2004年8月间在我院接受眼外伤后联合穿透性角膜移植的玻璃体视网膜手术20例(20眼),观察术后视力、角膜植片、视网膜脱离的复位情况和手术并发症.结果术后随访6月～3年,平均12±5.8月.末次随访期9眼(45.00%)视力优于术前.10眼(50.00%)角膜移植片保持透明,1眼(5.00%)角膜移植片发生严重的排斥反应,8眼(40.00%)角膜移植片出现浑浊水肿.3眼(15.00%)原视网膜脱离者术后再次出现视网膜脱离,其中1眼接受第二次玻璃体手术后视网膜复位.眼球萎缩2眼(10.00%).结论临时性人工角膜下玻璃体手术联合穿透性角膜移植术治疗合并角膜病变的视网膜玻璃体病变效果明显,恢复时间短.术前患眼的情况、前后段眼科医师手术技巧的熟练和配合程度是手术成功的关键.Objective To assess the efficacy of penetrating keratoplasty combined vitreoretinal surgery for traumatic eyes. Methods Twenty eyes from 20 consecutive patients, operated on between 2002.3 and 2004.8 and were followed up for at least 6 months. The evaluation focused on ocular history, visual acuity （VA）, anterior and posterior anatomical outcome, and complications. Results The average followup was 12±5.8 months （range 6-36 months）. At the final visit,VA increased in 9 eyes （45.00%）, conea remained clear in 10 eyes （50.00%）. A decrease in VA was caused by serious immunologic reaction in 1 eye （5.00%）, corneal graft edema in 8 eyes（40.00%）, recurrent retinal detachment in 3 eyes（15.00%） and ocular atrophy in 2 eyes （10.00%）. Conclusion The results of combined vitreoretinal surgery with penetrating keratoplasty is obvious. Preoperative ocular status, the skills and the cooperation of the anterior and posterior surgeons were the key to success.
