准分子激光原位角膜磨镶术治疗近视疗效分析

摘　　要：目的分析3年来准分子激光原位角膜磨镶术(LASIK)治疗近视的效果。方法对286例双眼或单眼屈光度在-0.50^-24.50D(等值球镜)的近视行LASIK手术治疗。术前按屈光度高低分3组:Ⅰ组近视程度-6.00D以内,Ⅱ组-6.12^-10.00D,Ⅲ组-10.00D以上。术前各组平均裸眼视力为:Ⅰ组0.2±0.08,Ⅱ组0.15±0.06,Ⅲ组0.08±0.06。术前各组平均屈光度分别为:Ⅰ组(-3.25±1.05)D,Ⅱ组(-7.32±1.15)D,Ⅲ组(-15.26±2.54)D。术后随访3年以上。结果术后裸眼视力和屈光度在1~6月内趋于稳定,术后3年3组平均裸眼视力为:Ⅰ组1.12±0.18,Ⅱ组0.95±0.18,Ⅲ组0.70±0.15,均远大于术前平均裸眼视力。3组平均屈光度3年后在±1.00D以内者依次为100.00%、93.64%和82.25%。结论LASIK治疗近视效果理想,其预测性稳定性较好,但术前严格筛选病例、熟练手术操作及积极预防和处理术后并发症是手术成功的重要保证。Objective To evaluate the three years outcome of excimer laser in situ keratomileusis（LASIK） for myopia with different diopters. Methods Three years follow up on 538 eyes of 286 cases undergoing Lasik （A Nidek EC-5000） were studied. According to the preoperative myopia ranging from -0.50 to -24.50D（spherical equivalent）, the eyes were assigned to three groups. Group A, 247 eyes ranging from -0.50 to -6.00D and group B, 204 eyes from -6.12 to -10.00D, group C, 87 eyes 〉 -10.00D, the means preoperative UCVA of three groups was 0.2 ± 0.08, 0. 154 ± 0.06, 0.08 ± 0.06; the means preoperative refraction of three groups was （-3.25 ± 1.05） D, （-7.32±1.15）D, （-15.26±2.54）D. Results The UCVA and refraction tended to be normal 1-6 months after operations. Three years after sugeries, the means UCVA was 1.12 ± 0.18, 0.95 ± 0. 18, 0.70 ± 0.15, more than preoprerative UCVA, the refration of 100%, 93.64% and 82.25% of eyes of three groups was in ranges of ＋ 1.00D. Conclusion LASIK is a safe, effective, predictable and stable procedure to correct myopia with different diopters.
