颞叶癫痫患者听觉功能初步研究

摘　　要：目的初步评价颞叶癫痫患者的听觉功能。方法对19例单侧颞叶癫痫患者(左侧10例,右侧9例),进行500Hz低通滤波言语测试、纯音测听、ABR及声导抗测试。选择20例健康人做对照组。结果纯音测听示实验组中纯音听阈均<25dB,分别与对照组相比,差异无显著意义(P>0.05);ABR检查中实验组与对照组在各波潜伏期、波间期差异无显著意义(P>0.05);实验组500Hz低通滤波言语识别率、“未滤-低通差”分别与对照组相比,差异无显著意义(P>0.05),但左颞叶癫痫患者组“未滤-低通差”右耳(69.7±12.3)%,左耳(67.1±10.3)%大于对照组右耳(61.7±8.3)%,左耳(60.1±8.3)%。结论实验结果未发现颞叶癫痫患者有明显听觉障碍。左颞叶癫痫患者“未滤-低通差”有增大趋势。OBJECTIVE To assess auditory function in patients with temporal lobe epilepsy.METHODS 500Hz low-pass filtered speech test, pure tone audiometry, ABR and acoustic immittance measurement were performed in 19 patients with temporal lobe epilepsy （ including 10 patients with left temporal lobe epilepsy and 9 patients with right temporal lobe epilepsy ） and 20 age-matched normal-hearing controls. All the patients with temporal lobe epilepsy were diagnosed by the neurologists prior to the tests. RESULTS There were no significant statistical differences in the threshold of pure tone average and tympanometry between the patients with temporal lobe epilepsy and normal subjects （P〉0.05）. In ABR test, no significant differences were found in the latency of waves and the interpeak latency intervals of waves between the groups （P 〉 0.05）. There were no significant statistical differences in the discrimination score for unfiltered speech and 500Hz low-pass filtered speech between the patients and the control group （P〉0.05）, but in difference between the unfiltered speech and low-pass filtered speech discrimination score, the score in the group of left temporal lobe epilepsy patients right ear （69.7±12.3）%,left ear（67.1±10.3）% was higher than that in the control group right ear（61.7±8.3）%, left ear （60.1±8.3）%. CONCLUSION The results showed that significant auditory dysfunction was not found in patients with temporal lobe epilepsy. But there was an upward trend in difference between the unfiltered speech and low-pass filtered speech discrimination scores in the left temporal lobe epilepsy patients.
