摘 要:背景:实验已证实金属硫蛋白对成熟心肌有保护作用,但对于未成熟心肌的保护作用尚缺乏实验证据。 目的:探讨诱导金属硫蛋白在未成熟心肌中的表达及对缺血再灌注未成熟心肌细胞功能的影响,为未成熟心肌的保护提供有效的方法。 设计:完全随机分组设计,对照实验。 单位:青岛大学医学院附属青岛市立医院妇产科和心脏外科,华中科技大学同济医学院附属协和医院心血管外科。 材料:实验于2001-10/2002-09在华中科技大学同济医学院药理实验室(安全级别一级)完成。选用出生14~21d日本长耳大白兔27只,雌雄不拘。随机将兔分为4组:对照组9只,腹腔注射ZnSO_4 12,24,48h组各6只。 方法:①对照组:腹腔注射蒸馏水0.3mL,按注射后12,24和48h取离体心脏,灌注重碳酸氢盐缓冲液15min转为工作心15min,全心停灌45min,恢复灌注15min改为工作心30min。腹腔注射ZnSO_1 12,24,48h组:分别按腹腔注射36g/L ZnSO_4 12,24和48h后取离体心脏,余方法同对照组。②计算心肌肌酸激酶和乳酸脱氢酶15min漏出率:心肌肌酸激酶(乳酸脱氢酶)(IU/L)×15min漏出液(L)/[1000×心肌湿重(g)]。③采用荧光素酶法心肌组织三磷酸腺苷含量。④心肌线粒体Ca^(2+)-三磷酸腺苷酶活性按Jones方法在Shimadzu UV-1200型紫外分光光度计上测定。⑤在电感耦合等离子体原子发射光谱仪上测定线粒体Ca^(2+)含量。⑥取制备的心肌线粒体悬液按Nayler法测定线粒体合成三磷酸腺苷能力。⑦采用^(109)Cd-血红素饱和法测定心肌组织中金属硫蛋白含量。组间计量资料差异比较采用t检验。 主要观察指标:各组兔心肌细胞中金属硫蛋白含量、肌酸激酶和乳酸脱氢酶漏出率、三磷酸腺苷含量、心肌细胞内Ca^(2+)含量、心肌线粒体Ca^(2+)-三磷酸腺苷酶活性及线粒体Ca^(2+)含量、心肌线粒体合成三磷酸腺苷�BACKGROUND: Metallothionein has been shown to possess protective effect on mature myocardium, but its protection of immature myocardium still needs support from experimental evidence.OBJECTIVE: To observe the expression of metallothionein in immature myocardium and assess its effect on the function of immature myocardium with ischemia-reperfusion injury, so as to provide an effective method for the protection of immature myocardium.DESIGN: Completely randomized and controlled experiment.SETTING: Department of Obstetrics and Gynecology, Department of Heart Surgery, Qingdao Municipal Hospital Affiliated to the Medical College of Qingdao University; Department of Cardiovascular Surgery, Xiebe Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology.MATERIALS: This experiment was carried out at the Pharmacological Laboratory (first security grade) of Tongji Medical College of Huazhong University of Science and Technology between January 2001 and September 2002. Totally 27 Japanese rabbits 14-21 days old of wither sex were randomized into 4 groups, namely control group (n=9) and ZnSO4 groups with sanpling time at 12, 24, and 48 hours, respectively (n=6).METHODS: The rabbits in control group were subjected to intraperitoneal injection of 0.3 mL distilled water and the heart was taken at 12,24 and 48 hours after the injection, respectively. The heart was perfused with bicarbonate buffer for 15 minutes and allowed to work for 15 minutes followed by perfusion withdrawal for 45 minutes, after which perfusion for 15 minutes was performed and the heart was again allowed to work for 30minutes. In the ZnSO4 groups, the rabbits were given intraperitoneal injection of 36 g/L ZnSO4 (dissolved in distilled water, 1.5 mL/kg) and the heart was taken at 12, 24 and 48 hours after the injection, respectively,followed by the same treatments as in the control group.
