扇型超声引导下经会阴前列腺穿刺活检的应用

摘　　要：目的探讨扇型超声引导下经会阴前列腺6针穿刺活检术诊断前列腺癌的价值。方法对可疑前列腺癌的104例患者,扇型超声引导下经会阴行双侧叶6点法穿刺活检术,对穿刺的阳性率和并发症及影响穿刺阳性率的因素进行分析。结果经病理诊断,检出前列腺癌24例,检出率23%,其余80例为良性前列腺增生(BPH)。术后未出现严重并发症。经分析发现在tPSA≥10μg/L、fPSA≥2μg/L、fPSA/tPSA<0.16、PSAD≥0.2和前列腺体积<40ml时是影响前列腺穿刺阳性率的重要因素(P<0.05)。结论扇型超声引导下经阴会6针前列腺穿刺活检,是一种安全准确的前列腺癌检出方法。Objective To evaluate the clinical value of ultrasound-guided transperineal 6-core prostate biopsy. Methods In a prospective study, 104 patients who had suspect of prostate cancer underwent transperineal 6-core prostate biopsy using the fan technique. The positive rate and the complication of this method was analysed. Results Prostate cancer was detected in 24 of the 104 patients （23%）. No severe complications could be found in this series. It was found that TPSA≥10 μg/L, fPSA≥2 μg/L, fPSA/tPSA≥0.16, PSAD≥0.2 and prostate volume〈40 ml are significant influence factors of biopsy positive rate （P〈0.05）. Conclusion 6-core transperineal prostate biopsy technique was proven to be an accurate and safe prostate biopsy method.
