中心性浆液性脉络膜视网膜病变激光治疗前后视觉对比敏感度的检测意义

摘　　要：目的探讨视觉对比敏感度(contrast sensitivity,CS)在中心性浆液性脉络膜视网膜病变(central serous chorioretinopathy,CSC)诊断、随访及疗效评价中的检测意义。方法选择45例CSC患者,随机分成激光治疗组27例,药物治疗组18例,观察两组患者治疗前后视觉对比敏感度的变化,随访6个月。结果激光治疗组患者CS(昼)在术后两周恢复者占70.4%,一个月内恢复者占96.3%,无一例复发;而药物治疗组患者CS(昼)在用药后两周恢复者占11.1%,一个月内恢复者占44.4%,复发率为16.7%。两组比较,有显著性差异,(P<0.05,P<0.01)。结论视觉对比敏感度作为一种全面、敏感衡量视功能的主要手段,在CSC的诊断、随访和疗效评价中有着重要的检测意义。在中心性浆液性脉络膜视网膜病变早期应用激光治疗可以明显改善视功能及提高治愈率。Objective To study the value of contrast sensitivity in diagnose, following-up and curative effect in central serous chorioretinopathy. Methods Among 45 patients with CSC, 27 patients were treated with laser, and 18 ones were treated with medicine as control group. To observe contrast sensitivity in two groups. Results The recovery rate of contrast sensitivity under the condition of glare in the laser treatment group was 70.4 % and 96.3 % after two weeks and after one month respectively and no patient recrude-sced. The recovery rate of the control group was 11.1% and 44.4% after two weeks and after one months respectively and the recurrence rate was 16.7 %. The difference was statistically significant （ P 〈 0.05, P 〈 0.01 ）. Conclusions The contrast sensitivity function test offers a more sensitive and comprehensive measure of functional vision. It plays a key role in diagnose, following-up and curative effect in central serous chori- oretinopathy. To treate CSC with laser can significantly improve visual acuity, recovery rate, especially at early stage of the disease.
