摘　　要：目的分析癫癎患儿苯巴比妥(PB)血药质量浓度的影响因素。方法对癫癎患儿83例进行103次血样检测,年龄1.5个月～16岁,采用高效液相色谱分析法测定其稳态血液谷质量浓度。采用多元逐步回归分析法分析。结果给药剂量、体质量对PB血药质量浓度影响有显著差异(P<0.05)。如果用CPB代表PB血药质量浓度(mg/L),D代表用药剂量(mg/kg),W代表体质量(kg),则多元回归方程为CPB=-1.711+4.194D+0.199W(r=0.661P=0)。结论尽管PB浓度受多种因索影响,但以用药剂量和体质量影响最明显,在无血药质量浓度监测条件医疗单位,大致可利用目标浓度和体质量来计算用药剂量。Objective To analyze the affecting factors in serum phenobarbital concentrations. Methods We measured serum phenobarbital concentration on total of 103 blood samples from 83 chidren with epilepsy, aged from 1.5 months to 16 years. The concentrations were measured by high proficiency liquid chromospectry（HPLC） in all the cases. Data were analyzed by multinomial linear stepwise regression of SPSS 11.5 version. Results It showed that only dosage and body weight had marked influence on serum phenobarbital concentration（ P 〈 0.05）. If we assigned CPB as serum phenobarbital concentration（mg/L）, D as dosage（mg/kg） and Was body weight（kg）, its multimonial formula was CPB=-1.711 ＋ 4.194D ＋ 0.199W （ r = 0. 661 P = 0）. Conclusions Although serum phenobarbital concentrations were influenced by many factors, among 24 possible factors, only phenobarbital dosage and body weight of children significantly affected serum phenobarbital concentration. Thus, clinicians can calculate phenobarbital dosage by target concentration and children＇s body weight if therapeutic drug monitoring was can＇t be performed in the grassroot hospitals.
