去髓操作中3种局部麻醉剂的综合比较

摘　　要：目的综合比较利多卡因、甲哌卡因和阿替卡因3种局麻药物在去髓操作中的临床指标。方法诊断为不可复性牙髓炎需行开髓、去髓和根管预备的患牙60个,随机分入A、B、C 3组,分别用利多卡因、甲哌卡因和阿替卡因进行局部麻醉,对起效时间、持续时间、麻醉效果和价格等进行比较。结果A、B、C 3组的起效时间分别为(3.05±2.14)min和(2.35±1.93)min,(2.80±1.70)min,以B组稍短;持续时间分别为(136.65±61.04)min,(154.05±25.79)min和(195.40±85.79)min,以C组稍长;麻醉优良率分别为9/20,10/20和15/20,以C组稍高;以上指标的差别均无显著性;3种麻药的单支进价分别为0.2、9.0和9.1元,以利多卡因最便宜。结论甲哌卡因和阿替卡因的临床效果较好,利多卡因最便宜。Objective To evaluate the three local anesthesia agents, lidocaine, mepivacaine and primacaine during the procedures of root canal therapy. Methods 60 teeth which suffered from irreversible pulpitis were randomly divided into three groups, A, B and C. One of the three localanesthesia agents was used separately before treatment. Results The beginning times were （3.05±2.14） min,（2.35± 1.93） min and （ 2.80 ± 1.70） min. The shortest group was B. The lasting times were （ 136.65 ± 61.04） min, （ 154.05 ±25.79） min and （ 195.40 ± 85.79） min. The longest group was C. Therefore, no differences in time were found between groups. The rates of excellenec were 9/20, 10/ 20 and 15/20. Group C was the best. The prices of the three local anesthesia agents were 0.2, 9.0 and 9.1 yuan, respectively. No significant differences in indexes above were found. Conclusion Mepivacaine and primacaine have excellent clinical effects, but lidocaine is the cheapest.
