摘  要:目的探讨直肠癌动脉血流阻力指数(RI)与微血管密度(microvessel density,MVD)的相关性及其与TNM分期的关系。方法63例直肠癌患者术前行经直肠多普勒超声检查,测量瘤内RI,术后确定肿瘤TNM分期,以抗CD105单克隆抗体行免疫组织化学染色,计数MVD,分析RI与MVD的相关性及其与TNM分期的关系。结果RI与MVD呈负相关(rs=-0.4225,P<0.01);各TNM分期RI比较Ⅰ期与Ⅲ、Ⅳ期,Ⅱ期与Ⅲ、Ⅳ期之间差异有显著性意义(P<0.05);各TNM分期之间MVD比较差异有显著性意义(P<0.05)。结论直肠癌RI可于术前反映肿瘤组织内血管生成活性,低RI反映了癌组织内高MVD状态。RI可间接反映直肠癌的病程进展,有一定的临床意义。Objective To explore whether resistance index (RI) correlates with the microvessel density(MVD) in rectal cancers and TNM stage. Methods Sixty-three cases with rectal cancer were examined preoperatively by transrectal Doppler ultrasonography, RI of arteries in rectal cancers were detected. After operation all cases were staged according to TNM standard. All the slices of tumor tissue specimens were stained immunohistochemistry with anti-CD105 monoclonal antibody and MVD were calculated separately. Relationships between RI, MVD and stages of the disease were analysed. Results There was negative correlation between RI and MVD( rs = -0.4225, P 〈0. 01). There were statistical differences of RI in different groups(TNM stage Ⅰ and Ⅲ , Ⅰ and Ⅳ, Ⅱ and Ⅲ , Ⅱ and Ⅳ) ( P 〈0. 05). There were significant differences of MVD in all TNM stages( P 〈0. 05). Conclusions RI of arteries in rectal cancers can evaluate the growing activity in tumor vessels. Lower RI showed higher MVD in rectal cancers. RI can reflect the development of rectal cancers indirectly and possesses a clinical meaning.
