摘　　要：目的:探讨进展期胃癌(AGC)手术及术中化疗等综合治疗的临床效果。方法:对136例AGC患者的临床资料进行回顾性分析。136例中Ⅱ期58例行胃癌标准根治(D2)术;Ⅲ期78例行胃癌扩大根治(D3)术,78例中13例行全胃切除“P”型空肠襻代胃Roux-en-Y术(PR),27例近端胃癌行联合脏器切除术。136例术中均进行静脉化疗和腹腔内温热化疗。结果:术后119例(87.5%)随访1-5年,Ⅱ期、Ⅲ期胃癌行D2、D2术后1、3、5年生存率分别为89.7%、64.5%、42.8%和83.7%,51.1%、35.1%。结论:AGC实施规范合理的胃癌根治术(D2、D3),并在术中应用静脉化疗和腹腔内温热化疗,可提高术后生存率。Objective： To investigate the clinical effect of gastrectomy and chemical therapy for advanced gastric carcinoma（AGC）. Methods： The clinical data of radical gastrectomy in 136 patients with AGC were analysed retrospective. 58 cases （stage of Ⅱ ） were accepted D2, 78 cases （stage of Ⅲ ） were accepted D3, the application of vein chemotherapy and intraperitoneal hyperthermia chemoperfusion to the all cases were in operation. Results： One hundred and nineteen patients （87.5%） had been followed up,the postoperative 1-, 3-, 5-year survival rate in D2, D3 were 89.7%, 64.5%, 42.8% and 83.7%0, 51.1%, 35.1% respectively. Conclusions： The patient with AGC should be perforemed standard operation. The vein chemotherapy and intraperitoneal hyperthermia chemoperfusion should be carried out during operation. The survival rate of the patient with AGC will be improved.
