摘 要:目的总结红外光经瞳孔温热疗法(transpupillary thermotherapy,TTT)治疗局限性脉络膜血管瘤的疗效.方法局限性脉络膜血管瘤患者8例8只眼,应用红外半导体激光治疗,波长810 nm,功率100 mW,光斑直径0.8~3.0 mm,曝光时间1 min.观察治疗前后视力、浆液性视网膜下积液的变化,彩色多普勒检查瘤体大小,瘤体检眼镜下所见及荧光血管造影中的渗漏情况及并发症等.随访16~40个月.结果经1次或重复的TTT治疗后,全部患眼的浆液性视网膜脱离平复,视力提高,彩色多普勒示瘤体消失.荧光血管造影复查可见瘤体渗漏减轻至消失,眼底检查可见轻微的脉络膜萎缩和视网膜色素上皮色素积聚.结论TTT是治疗伴或不伴有浆液性视网膜脱离的局限性脉络膜血管瘤的一种有效方法.Objective :To observe the clinical effects of transpupillary thermotherapy (TTT) on circumscribed choroidal hemangiomas. Methods The clinical data of 8 eyes in 8 patients with circumscribed choroidal hemangiomas with TTT were analyzed. Infrared diode laser with 1 minute of exposure time was used at 810nm and power at 100 mW with a beam diameter of 0. 8 to 3.0mm.The initial and final visual acuity( VA), the presence of serous subretinal fluid, the color Doppler appearance of the tumor, funduscopic appearance of the tumor, and the degree of hyperfluorescence observed during fluorescein angiography were the main parameters. Results :In all eight eyes treated with one or repeated applications of TTT, VA was improved and the hemangioma was disappeared in color Doppler, fluorescein angiography indicated the leakage of tumor vessels in the late angiograms was decreased or disappeared in all patients. Mild choroidal atrophy and minimal hyperpigmentation of the retinal pigment epithelium were observed in the treated eyes. Conclusions :TTT can be considered as an efficient therapeutic option for circumscribed choroidal hemangionms with or without serous retinal detachment.
