原发性肝癌肝阴虚证量化标准方法学研究

摘　　要：目的:建立原发性肝癌肝阴虚证量化标准(包括量化诊断标准和程度分级标准).方法:在流行病学调查的基础上,以肝阴虚证辨证参考标准进行辨证,根据相关因素在肝阴虚证组和非肝阴虚证组中出现状况的差异对相关因素进行赋分,并以此为基础建立量化诊断标准;然后对量化诊断标准进行检验;最后建立程度分级标准.结果:原发性肝癌肝阴虚证量化诊断标准为24分;量化诊断标准回归性和前瞻性检验的敏感度、特异度、准确度均在90%以上,阳性似然比分别为23.18和26.75;程度分级标准为:轻度24～32分,中度33～44分,重度≥45分.结论:同肝阴虚证各种形式的标准相比较,原发性肝癌肝阴虚证量化标准有以下几个特点:①更符合原发性肝癌的临床实际;②统计更严谨;③相关因素赋分更合理;④检验更严谨;⑤更具有临床实用价值.Objective： To set up a quantization standard （including quantization diagnosis standard and classification standard ） for deficiency of the liver-yin syndrome in primary liver cancer （PLC）. Method： Firstly, the quantization diagnosis standard for deficiency of the liver-yin syndrome was set up according to relevant referenced standard on the base of survey on clinical epidemiology, and its correlative factors was evaluated on the base of difference of their appearance between deficiency of the liver-yin syndrc：＇ae and non deficiency of the liver-yin syndrome. Secondly, the quantization diagnosis standard was tested. At last, the classification standard was established. Result： The metric diagnosis standard for PLC deficiency of the liver-yin syndrome was 24 points ; The sensitivity, specificity and accuracy of the prospective and retrospective examination of it were all higher than 90% ,posifive likelihood ratio of them were respectively 23.18 and 26.75. Twenty-four to thirty-two points was classified as the low grade, 33-44 points as the medium grade, ≥45 points as the high grade. Conclusion： Compared with those standards of deficiency of the liver, yin syndrome set up during studies in the past, the quantization standard for deficiency of the liver-yin syndrome of PLC has its characteristics： ①according with clinical characteristic of PLC; ②rigorous statistical methods; ③rationality of evaluation ;④religious examination;⑤better clinically practical value.
