17例前颅窝底大型脑膜瘤的外科治疗

摘　　要：本文报告了我院近期连续6年收治的前颅窝底大型脑膜瘤共17例。其中嗅沟脑膜瘤12例,其它部位5例,男9例,女8例,发病年龄多为30～60岁。首发症状多为头痛头晕,主要临床表现为嗅觉障碍,肿瘤直径均大于5cm。本组17例肿瘤全切,无手术死亡。术后病人均痊愈出院,随访6月～6年,效果满意。本文对前颅窝底大型脑膜瘤诊断和手术操作进行了介绍和讨论。This paper report our findings in 17 patients with large meningiomas of the front cranial fossa from May 1988 to December 1994. 12 Patients had olfactory groove meningiomas and 5 in other parts of the front cranial fossa. The most common Clinical manifestation was headache and impairment of smell.The impairment smell was usually ignored hy the patients themselves. A combined CT, MRI or cerebral angiography were made in all patients. The meningiomas in all patients were less than 5cm diameter. At operation, the following procedures were carried out:1. The entire tumor must be pulled forward and upward. 2. The tumor must be removed piecemeal. 3. The tumor root and peripheral vessels must also be removed. Microsurgical techniques were employed during the operation. The tumors of all 17 patients were totally resected and no patient died. Allpatients recovered well except those who had the poorest vision or were blind before the operation. The conclusion is that early diagnosis and proper operation are important factors affecting the prognosis.
