摘 要:目的探讨胆管癌组织中TGF-β1、TβRⅡ及p27kip1表达的相互关系及它们在胆管癌发生发展中的作用。方法应用免疫组织化学S-P法检测49例肝外胆管癌及8例正常胆总管组织中TGF-β1、TβRⅡ、p27kip1的表达,分析其与临床病理学特征的相关性。结果49例胆管癌组织TGF-β1、TβRⅡ和p27kip1阳性表达率分别为71.43%(35/49)、36.73%(18/49)和40.82%(20/49),而在正常胆管组织分别为12.50%(1/8)、100.00%(8/8)和87.50%(7/8);TGF-β1、p27kip1蛋白表达与胆管癌临床分期及转移相关,Ⅲ~Ⅳ期胆管癌的TβRⅡ阳性表达率(25.81%)明显低于Ⅰ~Ⅱ期(55.56%),P<0.05;TGF-β1及其Ⅱ型受体的共同表达与胆管癌临床分期相关(P<0.05);TβRⅡ与p27kip1在胆管癌中的表达呈正相关(γs=0.724,P<0.01);p27kip1表达与TGF-β1及TβRⅡ共同表达相关(P<0.01)。结论TGF-β1表达增高伴TβRⅡ、p27kip1表达降低提示胆管癌生物学行为不良;胆管癌中p27kip1蛋白的调控可能与TGF-β1信号转导有关。Objectives To investigate the mutual relationship between the expression of the TGF-β1 ,TβR Ⅱ and p27^kip1 in cholangiocarircinoma and its role in the development of cholangiocarcinoma. Methods Inmmnohistochemical S-P method was used to examine the expression of TGF-β1 ,TβR ]l and p27^kip1 in specimens of 49 cases of cholangiocarcinomas tissue and 8 cases of normal common bile duct. The relationship between their expression and clinicopathological characters was analyzed. Results The positive rates of TGF-β1 ,TβR Ⅱ and p27^kip1 were 71.43% (35/49) ,36.73% (18/49) and 40.82(20/49) respectively in cholangiocarcinoma, and 12.50% (1/8),100.00% (8/8) and 87.50(7/8) respectively in normal bile duct tissues. The expression of TGF-β1 and p27^kip1 was related to the carcinoma stage and metastasis. The positive expression rate of TβR Ⅱ was significantly lower in stage Ⅲ and Ⅳ disease than in stage Ⅰ and Ⅱ disease(25.81% v.s. 55.56 %, P 〈 0.05). The coexpression of TGF-β1 and TβR ]1 was negatively related with clinical stage(P 〈0.05). The p27kipl expression was positively associated with TβR-Ⅱ expression(γs = 0. 724, P 〈0.01). There was a significant relationship between the expression of p27^kip1 and the co-expression of TGF-β1 and TβR Ⅱ (P 〈 0.01). Conclusion The overexpression of TGFβ31 and lower-expression of TβR Ⅱ and p27^kip1 indicate poor biological behavior of cholangiocarcinoma. The regulation of the expression of p27^kiP1 may be related with the TGF-β1 signal transfer.
