摘 要:目的观察纯甘油保存的人羊膜用于原发性及复发性翼状胬肉治疗的临床疗效,并与细胞培养液(DMEM)加二甲亚砜(DMSO)保存的人羊膜进行比较,探讨简便实用的羊膜保存方法。方法对82例(90眼)翼状胬肉患者行翼状胬肉切除术联合羊膜移植术,随机对61例(65眼)联合使用纯甘油保存的羊膜(甘油组),包括45例(48眼)初发性翼状胬肉及16例(17眼)复发性翼状胬肉。21例(25眼)联合用细胞培养液加二甲亚砜保存的羊膜(DMEM+DMSO组),包括15例(18眼)初发性翼状胬肉及6例(7眼)复发性翼状胬肉。术后定期随访观察。结果术后随访7~29月,平均(16.2±6.3)月。共8眼复发,其中甘油组6眼复发,包括初发性翼状胬肉3眼和复发性翼状胬肉3眼。DMEM+DMSO组2眼复发,均为初发性翼状胬肉。2组复发率经χ2检验(χ2=0.061,P>0.05)差异无显著性,2组初发性翼状胬肉复发率(χ2=0.215,P>0.05)及复发性翼状胬肉复发率(Fisher’stest,P=0.333)均无统计学差异。结论纯甘油羊膜移植治疗初发性翼状胬肉及复发性翼状胬肉均取得满意疗效。纯甘油保存羊膜是一种安全、简便、有效的羊膜保存方法。Objective To investigate the effects of amniotic membranes preserved in the pure glycerin on the treatment of primary and recurrent pterygia, to observe the curative effects between the human amniotic membranes preserved in pure glycerin and in the Eagle' s solution containing demasob, and to find some simple but practical ways of preserving amniotic membranes. Methods The pterygial resection combined with amniotic membrane transplantation was performed on 82 pterygial patients (90 eyes). Sixty-one randomly chosen patients (65 eyes) were treated with amniotic membranes preserved in pure glycerin, including 45 primary pterygial patients (48 eyes) and 16 recurrent pterygial patients 07 eyes). Twenty-one patients (25 eyes) were treated with amniotic membranes preserved in Eagle's solution with demasorb , including 15 primary pterygial patients (18 eyes) and 6 recurrent pterygial patients (7 eyes). Results Eighty-two patients (90 eyes) was followed up 7-29 months( 16.2 ± 6.3 months). Recurrence of 8 eyes was observed, in which 6 eyes were in the glycerin group with 3 eyes of primary pterygia and 3 eyes of recurrent pterygia, the other 2 eyes in the Eagle's solution containing demasorb group with 2 eyes of primary pterygia and no eye of recurrent pterygia. There were no statistical difference in the recurrent rate ( x^2 = 0. 061, P 〉 0.05) between two groups. And no significant difference were found between two groups in the primary pterygial recurrent rate (X^2 =0.215,P〉0.05) and the recurrent pterygial recurrent rate (Fisher' s test, P = 0.333). Conclusion The treatment of the primary and recurrent pterygla by the transplantation of amntotlc membranes preserved in pure glycerin showed satisfactory curative effects.
