丝裂霉素C及可调整缝线在小梁切除术中的疗效分析

摘　　要：目的观察丝裂霉素C联合可调整缝线在青光眼小梁切除术中应用的效果。方法统计分析我院1995～2000年收治的青光眼患者应用丝裂霉素C联合可调整缝线治疗226例326眼,术中应用丝裂霉素C及可调整缝线1～2条。结果青光眼患者326眼,术后眼压控制在6～21mmHg(1kPa=7.5mmHg)者术后1周298眼,术后2周296眼,术后3月284眼,术后1a280眼,术后2a260眼,术后3a242眼。前房:第1d形成前房者318眼,6眼I度浅前房,2眼II度浅前房,均7d内恢复正常。滤过泡:随访2a,I型滤过泡96眼,II型220眼,III型8眼,IV型2眼。随访3a,I型滤过泡96眼,II型212眼,III型16眼,IV型2眼。结论丝裂霉素C可提高小梁切除术后眼压控制率,联合可调整缝线,可有效控制术后滤过水平,减少术后并发症,提高手术成功率。Objective To evaluate the curative effects of the application of mitomytin（MMC） and adjustable sewing line in the trabecnlar resection for the treatment of glaucoma. Methods Analysis of 326 eyes of 226 patients with glaucoma hospitalized in Anyang Eye Hospital from 1995 to 2000 was conducted.MMC and one or two adjustable sewing line were used in the trabecular resection, Results The post- operation, Introcular pressure（IOP） were controlled between 6 mmHg and 21 mmHg. One week after operative IOP of 298 eyes were kept the level;After two weeks there were 296 eyes;After three months there were 284 eyes;After one year there were 280 eyes;After two years there were 260 eyes;After three years there were 242 eyes. As for anterior chamber： 318 eyes had anterior-chamber in one day postoperatively including six eyes with Ⅰ° shallow anter chamber and 2 eyes with Ⅱ .All these eyes were recovered to normality in 7 days.Filter bulbs： after two years followup,96 eyes were I type filter bulbs, 220 eyes were Ⅱ type,8 eyes were Ⅲ type,2 eyes were Ⅳ type.After three years follow-up, 96 eyes were Ⅰ type filter bulbs,212 eyes were Ⅱ type, 16 eyes were Ⅲ type, 2 eyes were IV type. Conclusion The application of MMC can improve the IOP control after the trabecular resection. With the use of adjustable sewing line, filtering degree is effectively controlled and the syndrome obviously decreased.And thus the overall success of trabecnlar resection for treatment of glaucoma increases.
