晶状体半脱位患者的白内障超声乳化术疗效分析

摘　　要：目的 探讨超声乳化联合人工晶状体睫状沟缝线固定术治疗晶状体半脱位的疗效和技术。方法对23例(26只眼)晶状体半脱位患者，在应用2根细长针固定半脱位的晶状体后行超声乳化术，同时行人工晶状体睫状沟缝线固定术，观察术中及术后并发症及术后视力变化。结果 所有病例术后视力均有不同程度提高，术后1个月矫正视力处于0.1-1.0之间；0.1～0.3者10只眼，0.4～0.5者10只眼，大于0.5者6只眼。术中并发症主要是玻璃体脱出(19.2％)，术后并发症主要是角膜水肿(65.4％)、一过性高眼压(19.2％)、前房内及人工晶状体表面纤维素样渗出(23.1％)等，未见有视网膜脱离及角膜失代偿等严重并发症。结论 应用细长针固定半脱位的晶状体后行晶状体声超声乳化，同时行人工晶状体睫状沟缝合术，安全可行，手术效果良好，但要求术者有熟练的手术技术和丰富的临床经验。Objective To investigate the effectiveness and technique of the treatment of phacoemulsification and intraocular lens implantation on partly dislocated lens with artificial fixation . Methods Phacoemulsification and posterior chamber intraocular lens implantation with scleral fixation was performed on 23 patients （26 eyes） with partly dislocated lens. The complications in and after the operation and The postoperative visual acuity were observed. Results The best corrected visual acuity （BCVA） of all the patients was improved postoperatively. Pre- operatively, the visual acuity of all patients ranged from light - perception to 0.4, and among them 12 eyes （46.2%） 〈 0. 1,14 eyes were 0. 1 - 0.4. while postoperatively, their BCVA were 0.5 - 1.0 in 6 eyes , 0.1 - 0.5 in 20 eyes. The main complications included prolapse of vitreous （ 19.29% ）in the operation ,and corneal edema （65.4%） ,temporarily high intraocular pressure（ 19.2% ） and the bleeding of inter anterior chamber or the surface of the IOL（23.1% ）. No retinal detachment or cornea decompensation was observed. Conclusion The treatment of phaeocmulsifieation and IOL implantation on partly dislocated lens with artificial fixation is safe and effective. But it demands the operator have skillful operative technique and sophisticatedly clinical experiences.
