摘  要:目的探讨脑梗死(CI)及多梗死性痴呆(M ID)患者血清中S100β浓度的变化特点及临床意义。方法采用酶联免疫吸附法对62例CI患者及58例M ID患者血清中S100β浓度进行测定,并与40例脑供血不足组(正常对照)进行比较。智能水平采用智力低下状态检查(M M SE)进行评分,对<17分的参考H achinski缺血评分和美国精神病学诊断手册(DSM-IV)及国立神经病学、语言功能障碍和中风研究所(N INCD S-ADR DA)诊断标准进行诊断。结果M ID急性发作组血清S100β的浓度明显高于M ID稳定期组和对照组(P<0.05)及CI急性期组,在24~48 h、第7天、第15天差异均有统计学意义(P<0.01);M ID稳定期组S100β的浓度显著高于CI稳定期组和对照组(P<0.01);CI急性期组其浓度明显高于CI稳定期组和对照组(P<0.05),而CI稳定期组与对照组比较差异无统计学意义(P>0.05);M ID各组血清中S100β浓度与智能状态无直接相关性(P>0.05)。结论血清S100β的含量变化可动态反映M ID患者的脑组织功能状态,但并不是一种智能因子,不能直接反映M ID患者的智能水平。Objective To probe into the change characteristics of serum concentration of S100β and its clinical significance in patients with multi-infarct dementia(MID) and cerebral infarction(CI).Methods Serum concentration of S100β was determined by ELISA in 58 patients with MID and 62 with CI,and compared with normal control group.Mini-mental state examination (MMSE) was used to mark the intellectual level of patients,for those under 17 points the Hachinski Ischemia Score, DSM-IV and NINCDS-ADRDA diagnosis standard were used as reference.Results Serum concentration of S100β in MID acute episode group was significantly higher than that in MID stationary phase group and that in normal controls (P〈0.05),and was significantly higher than that in CI acute phase group respectively on the 24-48 h,7th day,15th day (P〈0.01);the concentration of S100β in MID stationary phase group was significantly higher than that in CI stationary phase group and that in normal controls (P〈0.01);its concentration in CI acute phase group was higher than that in CI stationary phase group and that in normal controls (P〈0.05).No statisical difference in expressions existed between CI stationary phase group and control group(P〉0.05);There was no direct correlation between blood S100β concentration and intelligent state (r=-0.0352,P〉0.05 ;r=-0.0634,P〉0.05).Conclusion The change of serum levels of S100β could dynamically reflect the brain functional status of MID, however it could not directly indicate the intelligence of patients,for S100β was not an intellectual factor.
