肝癌组织端粒酶逆转录酶hTERT基因及GGT-mRNA-H亚型表达与肝癌早期诊断的研究  被引量:7

摘  要:目的研究肝癌组织中端粒酶逆转录酶hTERT基因及谷氨酰转移酶GGTmRNA H亚型表达,探讨其在肝癌早期诊断中的意义。方法应用RT PCR法,检测65例原发性肝癌患者癌组织及癌周组织、108例良性肝病肝组织、6例健康对照肝组织的hTERT与GGTmRNA H亚型,观察这两种基因的表达情况。结果在65例原发性肝癌中,癌组织hTERT阳性56例(86.2%),GGTmRNA H亚型阳性61例(93.8%),AFP阳性40例(61.5%)(P<0.05);hTERT在20例小肝癌癌组织阳性表达16例(80.0%),GGTmRNA H亚型阳性表达18例(90.0%),与健康对照组差异有统计学意义(P<0.05)。急慢性肝炎组、肝硬化组与健康对照组比较,端粒酶逆转录酶、GGTmRNA H亚型阳性表达率虽明显升高,但其差异无统计学意义(P>0.05)。结论肝癌组织中hTERT、GGTmRNA H亚型联合分析可弥补AFP检测的不足,对提高肝癌、癌前病变的早期诊断具有重要意义。小肝癌中端粒酶活性增高在癌形成中扮演重要的角色。hTERT基因检测比AFP更敏感、特异性更高,有望成为肝癌早期诊断的理想指标。Objective:TO determine the relationship between the expression of hTERT and GGTmRNA-H gene in HCC and patient's prognosis. Methods :The expression of HTERT and GGT- mRNA-H type was examined using RT-PCR method in cancerous tissues and noncancerous tissues adjacent to tumors in 65 patients with HOC,in 108 patients with benign liver diseases,and 6 normal control persons. Results:In 65 Patients with primary liver cancer, teuomerase transcriptase was expressed in 56 patients(86. 296),GGTmRNA-H was in 61 patients(93. 896),and serum AFP was POsitive in 4 0 patients ( 6 ] . 5 % ). Telornerase transcriptase was positive In 16 patients with small liver cancer,the positive rate was 80.0%. The positive expression of GGTmRNA subtype were 90.0% (18/20). Althoueh the positive expression rates of telomerase transorlptase and GGTmRNA-H were hi6her in patients with aoute and chronic hepatitis, liver cirrhosis oompared with normal oontrol groups, there was no statistical sisniflcanoe(P〉0.05). Conclusion:The oomblned examination of GGTmRNA-H subtype and telomerase transoriptase can avoid the disadvantase of AFP examination,and has gteat importanoe in early diagnosis of liver cancer. The rising of telomerase aotivity in small liver canoer plays an Important role in the canoeratlon prooess. The result of hTERT examination is more sensitive and more gpeolfio than AFP examination,and would be an Ideal tumor marker for the early diagnosis of HCC.
