动态观测胃粘膜不典型增生MC5抗原表达与临床随访研究

摘　　要：应用本院制备的抗结肠癌单克隆抗体ＭＣ５及ＡＢＣ免疫组化染色检测了６４例具有随访结果的胃粘膜异型增生标本。其中癌变组３０例，未癌变组３４例，随访时间６～７２个月（平均２６个月）。结果：癌变组中有２０例表达阳性（６６．７％），未癌变组中有８例表达阳性（２３．５％），两组相比相差显著（Ｐ＜０．０１）。表明ＭＣ５－Ａｇ表达阳性的不典型增生与胃癌关系密切。在未癌变组ＭＣ５表达阴性的２６例标本中，随访后病变减轻或无变化者２３例，ＭＣ５仍呈阴性表达，而未癌变组ＭＣ５表达阳性的８例中仅一例病变减轻（Ｐ＜０．０１）．提示ＭＣ５抗原表达阴性的病例具有可逆性。本研究将为进一步对ＭＣ５－Ａｇ定量、寻找高危界值、建立癌变概率差别模式提供资料。By using ABC immunohistochemical method,we study the expression of MC5-Ag in 64 cas-es of dysplcqsia of gastric mucosa which had been followed-up.Among the 64 cases of dysplasia,iin 30 casescancer had been found,in the others 34 cases no cancer had been found.The time of following-up was 6-72 months(average 26 months).The expression of MC5-Ag were positive in 20 out of 30 cases with cancer(66.7%),only 8 out of 34 cnses with no cancer. There was a significant statistical difference between twogrotip(P <0.01).Reversally it was found that in 23 out of 26 cases of no cancer group with MC5 negativestaining, only one cases in no cancer group with MC5 positive staining(P<0.01).These finding showedthat the positive expression of MC5-Ag may predict the possibility of oncogenesis in patients with dysplasiaof geastric mucosa. If a closed following-up was performed in these patients,it may be possible to detectearlier gastric cancer.
