电视胸腔镜手术的临床应用

摘　　要：在电视胸腔镜（VATS）下行自发性气胸肺大泡结扎加胸膜粘连术8例，肺边缘肿瘤切除2例，原因不明的胸腔积液的诊断与治疗2例，纵隔肿瘤摘除术1例，脓胸清创引流术2例。本组15例中，1例右侧原发性慢性脓胸，需作大面积胸膜纤维板剥脱，为达到1期效果，中转前外侧开胸外，其余14例进行顺利，都取得了满意效果。表明此术式安全有效，损伤小，术后并发症少，恢复快。This paper reported video assisted tboracoscopic surgery(VATS) on 15 cases of chest diseases. 8 cases of sponta-neous pneumothorax undderwent VATS for stapled bullectomy, 2 cases of marginal pulmonary carcinoma for resection, 2 cases ofPleural effusion of unknown cause for diagnosis and treatment, 1 case of mediastinal tumor for excision, 2 cases of empyema forepluchage and Jrainage. All Procedures were Smooth and satisfactory except in 1 case of ptimary chronic empyema that the VATSwas converted into thoracotomy to remove large area of pleural fibrous Plate in order to get healing by first intention. It was foundthat these thoracoscopic surgical procedures were safe,effective.and less traumatic,pain and blood loss were lessened only a fewpostoperative complications had been encountered. and the average length of hospital stay was shortened because of quick recov-ery. It Is a valuable procedure for clinical use in chest diseases.
