新鲜羊膜移植治疗难治性角膜溃疡的临床效果

摘　　要：目的探讨羊膜移植在难治性角膜溃疡中的临床治疗效果.方法采用新鲜羊膜移植治疗难治性角膜溃疡共12例(12眼),其中细菌感染3例,真菌感染3例,病毒感染2例,蚕蚀性角膜溃疡4例.经相应抗感染药物治疗无好转,行单层或多层羊膜覆盖术.结果全部患者角膜感染均于术后5～15天控制,角膜溃疡于术后4～5周角膜上皮愈合,溃疡区留下不同程度的瘢痕.视力手动～0.1者7眼,0.3～0.6者5眼.结论新鲜羊膜移植治疗药物难以控制的角膜溃疡是一种有效的方法.Objective To evaluate tile therapeutic effect of amniotic membrane transplantation（AMT） in corneal ulcer refractory to conventional treatment. Methods Fresh AMT was performed in 12 patients （12 eyes） of refractory corneal ulcer. There were 3 eyes with bacterial corneal ulcers, 3 eyes with fungal corneal ulcer, 2 eyes with herpes simplex keratitis, 4 eyes of Mooren＇ s ulcer without specifil microorganism were treated with AMT. Results No actue rejection was observed after fresh AMT in all patients. The infection or inflammation were controlled in 5-15 days after surgery, in 11 of 12 eyes epithelium healed over all corneal ulcer within 4-5 weeks and remained stable during the follow up. The uncorrected visual acuity was from 0.3 to 0.6 in 5 eyes and handmovement to 0.1 in 7 eyes. Conclusion Fresh AMT is a very effective method for the treatment of refractory corneal ulcer.
