翼状胬肉三种手术方法的临床效果观察

摘　　要：目的对比3种手术方法治疗翼状胬肉的临床效果.方法翼状胬肉63例(63眼)随机分为3组:第1组,胬肉单纯切除;第2组,自体角膜缘上皮联合球结膜移植;第3组,翼状胬肉切除球结膜移植术中应用丝裂霉素C.每组均为21例.观察术后角膜愈合情况、角膜缘新生血管及球结膜是否有结膜翼状胬肉样组织增生.结果随访6～24月.第1组中有7例复发(33.33%);第2组及第3组均无复发者.结论单纯手术切除不能从根本上阻止翼状胬肉复发,自体角膜缘上皮联合球结膜移植术及翼状胬肉切除球结膜移植术中应用丝裂霉素C均可有效地阻止胬肉复发.Objective Compared the clinical effect of simple excision of pterygium, limbal epithelium and conjunctiva autografting combined excision, conjuctiva autografting and mitomycin treatment during operation combined excision, Methods 63 cases （63 eyes ） with ptergium were divided into three groups randomly. The first group were treated with simple excision. The second group were treated with limbal epithelium and conjunctiva autografting combined excision. The third group were treated with conjunctiva autografting and mitomycin treatment during operation combined excision. Cornea healing, limbal neovascular and tissue like pterygium hyperplasia were observed. Results Following up was 6-24 months, 7 cases （7 eyes） had pterygium recurrence in the first group （21 cases）. The recurrence rate was 33. 33% ,There wasn＇t recurrence in the second and the third group, Conclusion Simple excision of pterygium can＇t effectively deal with recurrence. Conjuctiva auto grafting and mitomycin treatment during operation combined excision, limbal epithelium and conjunctiva autografting combined excision are two effectively methods for preventing recurrence.
