摘  要:目的研究急性白血病(AL)细胞DNA倍体及增殖细胞核抗原(PCNA)的变化,探讨AL细胞DNA倍体与白血病细胞增殖的关系及临床意义。方法图像细胞术分析41例AL细胞DNA倍体的变化;单克隆抗体免疫细胞化学染色技术检测42例AL细胞PCNA的表达。结果对照组细胞DNA含量以2C为主,未发现DNA含量≥5C的多倍体细胞;DNA含量分布范围较窄,倍体峰相较单一。AL组≥5C的多倍体细胞明显增加,DNA含量分布范围较宽,倍体峰相右移或多峰相。AL、ALL和ANLL的多倍体检出率分别为56.1%,62.5%和52.0%,均明显高于对照组(P<0.01)。对照组PCNA表达阴性,AL、ANLL和ALL的PCNA阳性率分别为59.5%,62%和56%;DNA多倍体阳性组PCNA阳性细胞数(47.6%±20.76%)明显高于DNA多倍体阴性组[(4.61%±11.01%),P<0.01]。结论AL细胞PCNA表达和DNA合成及细胞的恶性增殖潜能密切相关;图像分析技术可同步进行AL细胞DNA测定和形态观察;通过倍体直方图可直观、简便地观察DNA多倍体情况。Objective To explore the DNA ploidy in acute leukemic cells and its relation to the expression of proliferation cell nuclear antigen(PCNA). Methods The DNA content in 41 acute leukemia(AL) was measured by image cytometry(ICM). The PCNA expression in 42 AL was analyzed by immunocytochemistry. Results The DNA distribution figures indicate that most of DNA content in the control consisted of 2C while those in the AL groupe consisted of 5C or more mainly; the DNA content in the control covered a relatively narrower area while those in the AL covered a relatively wider area. The DNA aneuploidy in AL, ALL and ANLL was found 56.1%, 62.5 % and 52.0 % respectively. The rate was found significantly higher than that in the control(0%, P〈0.01). The PCNA positive rate in AL, ANLL and ALL was found 59.5%, 62% and 56% respectively. The percentage of PCNA positive cells in AL with DNA aneuploidy (47.6%±20.76 % ) was significantly higher than that in AL with DNA non-aneuploidy [ (4.61% ± 11.01%), P 〈 0.01 ]. Conclusion DNA content and morphology can be analyzed at the same time, DNA aneuploidy could be observed directly and conveniently by image cytometry in acute leunemic cell.
