摘 要:目的测定肿瘤患者血浆中的二氢尿嘧啶(H2U)与尿嘧啶(U)的比值来间接估计二氢尿嘧啶脱氢酶(DPD)的活性.方法采用HPLC法测定肿瘤患者血浆中内源性物质H2U和U的比值.色谱柱:反相C1s柱(300mm×4.6 mm,5 μm);流动相:0.01 mol·L-1磷酸盐缓冲液(pH3.0),5 μmol·L-1磷酸三乙胺;检测波长204 nm.结果 202例肿瘤患者入选,其H2U/U为(2.50±1.25)(0.53~7.51),男性为(2.58±0.85),女性为(2.41±0.98);有50例的H2U/U低于2.0,其中男性患者为29例,女性为21例.DPD酶活性与年龄、性别不存在相关性(P>0.05).结论通过HPLC法测定血浆DPD酶活性来调节氟尿嘧啶的剂量,从而达到个体化给药的目的.AIM To determine the ratio of dihydrouracil (H2U) and uracil (U) in blood of patients with cancer, and to predict the activity of dihydropyrimidine dehydrogenase (DPD) directly. METHODS The concentration of U and its dihydrogenated metabolite (H2U) were simultaneously determined by HPLC. The liquid chromatography column was ODS C18 (300 mm× 4.6 mm, 5 μm); the mobile phase was 0.01 mol· L^-1 phosphate buffer ( pH 3.0), 5 μm triethlamine phosphate;the wave length was 204 nm.The ratio of H2U and U was calculated. RESULTS Two hundred and two patients were enrolled. The H2U-U ratios were Gaussian distributed [ mean value (2.50 ± 1.25 ), range 0.53 - 7.51 ]. Fifty patients' H2U-U ratios were lower than 2.0 (female:21 ,male:29).The H2U-U ratios were no significant difference according to sex ( P 〉 0.05) ( male 2.58 ± 0.85, female 2.41 ± 0.98 ; respectively), and also no significant difference according to age ( P 〉 0.05). CONCLUSION The H2U-U ratios, easily determined by HPLC before treatment, could help to identify patients with metabolic deficiency, and then to adjust the dosage of 5-FU.
