摘 要:目的研究P53、Ki-67、PCNA在宫颈鳞状上皮不典型增生、鳞状上皮原位癌及浸润性鳞状上皮癌中的表达及意义. 方法用免疫组化方法(S-P法)检测9例慢性宫颈炎、6例尖锐湿疣、24例宫颈鳞状上皮不典型增生、13例原位癌、5例早期浸润癌和33例浸润性宫颈鳞状上皮癌P53、Ki-67、PCNA的表达情况. 结果 P53在慢性宫颈炎,宫颈尖锐湿疣,不典型增生,原位癌及浸润性鳞状上皮癌之间无明显差异性,但与癌组织分化程度有关(P<0.05)且早期浸润癌与浸润性鳞癌之间差异非常显著(P<0.01),Ki-67的表达与组织分化程度有关(P<0.05),PCNA在慢性宫颈炎中无明显表达,在尖锐湿疣、不典型增生及癌中均有93.3%~100%的表达,且与组织分化程度无关. 结论 P53异常表达是宫颈癌的早期事件,与宫颈癌发生有关,PCNA和Ki-67在宫颈鳞状上皮不典型增生及癌中均有高表达,表明它们在宫颈鳞状上皮癌的发生及发展中起作用.Objective To study the expression and significance of p53,Ki - 67 and PCNA in cervical squamous intraepithelial neoplasm squamous cell carcinoma in situ and invasive squamous cell carcinoma. Methods The circumstance of expression of P53 ,Ki - 67 and PCNA in 9 examples of chronic eervieitis ,6 examples of eondyloma acuminatum ,24 examples of cervical squamous intraepithelial neoplasm, 13 examples of carcinoma in situ, 5 examples of early invasive carcinoma and 33examples of invasive cervical squamous cell carcinoma were determined with immunohistochemistry method (S - P method). Results No significant differences in the expression of P53 in the patients of chronic eervieitis 、 cervical eondyloma aeuminatum, squamous intraepithelial neoplasm, carcinoma in situ and invasive squamous epithelial carcinoma were observed. But it was associated with tissue differentiation degree ( P 〈 0.05) and early invasive carcinoma and invasive squamous carcinoma (P 〈 0.01). The expression of Ki - 67 was associated with tissue differentiation degree (P 〈 0.05). PCNA did not apparently expressed in chronic eervieitis, but there a 93.3% - 100% expression in eondyloma aeuminatum, intraepithelial neoplasm and squamous carcinoma was noticed. It was associated with tissue differentiation degree. Conclusion The over expression of P53 is the earlier sign of cervical carcinoma and it has something to do with the occurrence of cervical carcinoma . PCNA and Ki - 67 all highly expressed in cervical squamous intraepithelial neoplasm and squamous carcinoma, showing that they play a role in the occurrence and development of cervical squamous cell carcinoma.
