大龄单纯腭裂患者上颌骨矢状方向生长发育的研究

摘　　要：目的研究大龄未手术单纯腭裂患者上颌骨前后向(矢状方向)的生长发育。方法大龄(>16岁)单纯腭裂未手术患者25例,拍摄头颅定位侧位片,测量分析矢状方向的线距和角度。以同龄单纯腭裂手术后患者和正常者作为对照,采用ANOVA法对所测数据进行比较分析。结果腭裂未手术组与正常组比较,存在明显的上颌后缩、上颌长度不足以及下颌前突,呈现安氏Ⅲ类倾向。该特征在腭裂术后患者中同样存在。结论单纯腭裂患者上颌骨呈现矢状方向的生长发育缺陷,可能与畸形本身的内在因素有关。Objective To study the anteroposterior maxillary growth and development of adult unoperated isolate cleft palate patients and to describe the anteroposterior facial morphology eliminating the influence of cleft management, and also to assay the palate surgical effects on jaw development. Methods Lateral cephalograms of 25 adult unoperated isolate cleft palate patients were traced and compared with those of cleft operated patients and normal individuals. The results of various measurements in the anteroposterior aspect were analyzed with ANOVA. Results In adult unoperated isolate cleft palate patients, the results showed a short maxilla as well as a retruded maxilla with C-Ⅲ tendency. These characters were also observed in the the operated cleft individuals. Conclusion There are inhibitory effects of anteroposterior growth and development of maxilla in cleft palate patients, which may result from intrinsic defects in cleft patients.
