摘 要:目的观察中国人脂联素基因(adipase most abundont gene transcript1,APM1)SNP276多态性分布及其与2型糖尿病及胰岛素敏感性的关系。方法417名无亲缘关系的安徽地区汉族人,141人为正常糖耐量(normal glucose tolerance,NGT)者,276例为2型糖尿病(type2diabetes,T2DM)患者。用稳态模式胰岛素抵抗指数(HOMA-IR)及胰岛β细胞功能指数(HOMA-βcell)估测胰岛β细胞胰岛素分泌功能及外周组织胰岛素敏感性。采用聚合酶链反应-单链构象多态性方法初筛APM1后,进行DNA直接测序。结果(1)NGT者SNP276基因型分别为GG0.489、GT0.418及TT0.092,G为优势等位基因,频率为0.699;SNP276基因型及等位基因在NGT与T2DM者间分布差异有统计学意义(P值为0.031,0.013),GG+TG基因型与HOMA-IR正相关(r=0.199,P<0.01),其携带者患T2DM的风险增加(OR=2·447,95%CI1.067-5.612,P=0.035)。(2)T2DM组中,无论GG型或GT型,与TT型相比,前者的体重指数、体脂百分比、空腹血糖及HOMA-IR均高于后者(P<0.05或<0.01),另外,GG型的收缩压亦高于TT型(P=0·011);NGT组中,可见相似结果,非TT型的体重指数、体脂百分比、腰臀比、空腹胰岛素、口服葡萄糖耐量试验2h胰岛素及HOMA-IR均高于TT型(P<0.05或<0.01)。结论APM1基因SNP276多态性与中国人2型糖尿病及胰岛素敏感性具有相关性。Objective To investigate the distribution of SNP276 in adiponectin gene in Chinese Hans and its impact on type 2 diabetes mellitus and insulin sensitivity. Methods The study population consisted of 417 Chinese Hans residents in Anhui province, including 141 subjects with normal glucose tolerance (NGT) and 276 with type 2 diabetes (T2DM). The islet beta-cell insulin secretion and tissue insulin sensitivity were assessed by formulae of homeostasis model assessment (HOMA-IR & HOMA-β). Firstly, polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) was used to determine whether variation exists in APMI. Then, exact variation was detected by automated DNA direct sequencing. Results The genotypes of APMI SNT276 were 0.489 GG,0.418 GT and 0.092 TT and the major allele was G( frequency = 0. 699)in subjects with NGT. The distributions of genotypes and alleles of SNF276 both displayed significant difference between NGT and T2DM groups ( P = 0.031 and 0.013 ). The SNP276 non- TT ( TG + GG) genotype was associated with increased risk of T2DM( OR = 2. 447,95 % CI 1. 067-5. 612, P = 0. 035 ). In T2DM group, the subjects with SNP276 GG or GT geuotype had higher body mass index, body fat content, fasting plasma glucose and HOMA-IR than did those with TT genotype (P 〈 0.05 or P 〈 0.01 ). Besides, GG genotype had higher systolic blood pressure( P = 0.021). In NGT group, SNP276 non-TT carrier had increased body mass index, body fat content, waist hip ratio, fasting plasma insulin, oral glucose tolerance test 2 h plasma insulin and HOMA-IR when compared with TT genotype ( P 〈 0.05 or 0.01 ). Conclusion SNP276 in APMI was associated with T2DM and insulin sensitivity.
