利用种植型牵张器增高兔下颌牙槽嵴的实验研究

摘　　要：目的介绍自行研制的种植型骨牵张器的牵张成骨效果,探讨其在动物实验中的应用方法与可行性.方法日本大耳白兔8只,体重2.0～2.5kg,随机分4组,每组2只,随机选取一侧兔下颌牙槽嵴,矩形截骨,植入种植型骨牵张器,延迟5天后垂直牵张加高牙槽嵴,l次/d,1.5 mm/次,共加力牵张3d,分别于牵张后1周、2周、4周、6周处死各2只兔子,并进行X线和组织学观察.结果除1枚种植型牵张器因伤口感染发生松动而取出外,其余牵张器与周围组织均愈合良好,牙槽嵴平均加高4.00mm,X线显示牵张6周时牵张间隙消失,组织学观察6周时牵张间隙被成熟新生骨修复.结论种植型骨牵张器可用于垂直牵张加高牙槽嵴.Objective To introduce the new type of implant-distraction apparatus and to investi- gate the fesitability of apparatus through an experimental studying in rabbits. Methods A total of eight rabbits were randomly divided into four groups, each group consist of two rabbits. A box-shaped osteotomy on the random side of the alveolar bone was carried out and a implant-distraction apparatus was placed. After five days, the alveolar ridge was vertically augmented at 1.5 mm/d by distraction for 3 days. At every of I week, 2 weeks, 4 weeks and 6 weeks, two rabbits were killed , and histological and radiogaphical evaluations were carried out. Results All dental implant-distraction appa- ratus obtained good healing except one got inflammation and was removed. The alveolar ridge was vertically augmented 4.00 mm on average.The radiolucency in the distraction site disappeared in 6 weeks. Histologically, the lengthened bone consisted of mature lamellar and canellous bone. Conclusions The dental implant-distraction apparatus may be used for vertical augmentation of the alveolar rictus.
