摘  要:目的探讨hTERT基因表达在皮肤鳞状细胞癌(鳞癌)进展过程中的作用。方法收集10例正常皮肤、11例假上皮瘤样增生(PEH)、66例皮肤鳞癌原发灶及12例淋巴结转移灶标本和临床、病理学资料,采用原位杂交法检测hTERT mRNA,结果以图像分析方法定量分析。结果hTERT mRNA在正常皮肤、PEH及鳞癌中的阳性系数(PU)分别为0.84±0.58、3.06±1.09和10.01±0.60(P<0.01),鳞癌与前两者间差异有非常显著性(P<0.01);高分化与低分化组PU值分别为8.52±0.63和13.42±1.02,差异有非常显著性(P<0.01);转移组与无转移组、已发生转移病例原发灶与转移灶间差异均无显著性(P>0.05)。结论与正常皮肤、PEH相比,鳞癌中hTERT基因表达明显增高,分化越低表达越强,提示端粒酶激活在鳞癌恶变过程中起了关键作用。Objective The purpose of this study was to investigate whether the expression of hTERT correlates with progression and metastasis of cutaneous squamous cell carcinoma(CSCC). Methods The hTERT mRNA was detected by in situ hybridization (ISH) in paraffin- embedded specimens of 10 normal cutaneous tissues, 11 pseudoepitheliomatous hyperplasias (PEHs), 66 primary CSCC s and 12 lymphaden metastases. Meanwhile, All of the results were quantitively measured by morphometry followed statistical analysis. Results The positve units (PU) of hTERT mRNA staining in normal, PEH and CSCC were 0.84 ±0. 58,3.06 ± 1.09 and 10.01 ± 0.60 respectively. There was a significant ( P 〈 0.01 ) difference between CSCC and others. Well differentiated CSCC,as compared to poorly differentiated CSCC,exhibited a significant (P 〈 0.01 ) weaker staining for hTERT mRNA, whereas no significant difference was found between metastatic and non-metastatic group, or between primary tumors and the matched lymphaden metastases. Conclusion hTERT expression was increased in CSCC, the more poor differentiated, the more stronger expression, which implied that telomerase reactivation had a crucial role in malignant transformation of CSCC,our data suggest that hTERT can be used as a molecular progression marker when screening for CSCC and have some values in clinical practice.
