小骨瓣纵裂入路显微外科手术切除大型及巨大型垂体瘤

摘　　要：目的探索大型及巨大型垂体腺瘤根治性切除的手术入路、方法及效果.方法 22例大型及巨大型侵袭性垂体腺瘤采用小骨瓣经纵裂显微外科手术,大型者肿瘤最大径3.5～4 cm,巨大型侵袭性肿瘤最大径4.2～5.7 cm.结果大型8例全切7例,近全切1例;巨大型肿瘤全切11例,3例大部切除.全部病例术后无脑脊液漏,无严重并发症,无死亡.结论小骨瓣纵裂入路显微外科手术切除大型及巨大型垂体腺瘤是对脑组织损伤小、并发症少、全切率高的手术方法.Objective To study the operative approach, surgical technique and their effects on treating large or giant invasive pituitary adenomas. Methods Twenty-two cases of large and giant invasive pituitary adenomas were operated on by the longitudinal interhemispheric approach with little bone flap. The diameter of large tumors was 3.5～4.0 cm and the diameter of giant tumors was 4.2～5.7 cm. Results Total removal was got in 18 cases, subtotal removal was got in 4 cases, without death or severe complications. Conclusion Longitudinal interhemispheric approach is an ideal method for surgery of giant invasive pituitary adenomas.
