颅脑损伤所致智能障碍的影响因素分析

摘　　要：目的:探讨颅脑损伤后司法精神病学鉴定为智能障碍的特点及其影响因素。方法:对185例颅脑损伤患者的司法精神病学鉴定资料进行回顾性分析,颅脑损伤诊断按照病史资料和颅脑CT/MRI确定;智能障碍按照中国精神疾病分类方案与诊断标准第2版修订本、中国精神障碍分类与诊断标准第3版(CCMD-2-R、CCMD-3)及修订的韦氏成人智力量表(WAIS-R)确定;所得结果进行单因素及多因素回归分析。结果:185例中智能障碍者为69例,占37.3%;有智能障碍组的受伤时昏迷时间、神经系统阳性体征及生活自理功能障碍比例高于无智能障碍组(P<0.05);多因素回归分析进入方程的因素依次是:生活自理功能障碍、神经系统阳性体征、脑叶损伤范围。结论:颅脑损伤后鉴定为智能障碍的影响因素依次包括颅脑损伤后生活自理功能障碍、神经系统阳性体征、脑叶损伤范围。Objective：To explore the character and related influential factors of the intellectual impairment due to brain injury in forensic assessment. Method：Retrospectively investigated and analyzed the clinical data of the patients （n = 185） who were accepted the forensic assessment because of brain injury. According to the results of the clinical feature and the brain CT/MRI, giving the diagnosis of brain injury. Furthermore, the intellectual impairment criteria is according to the Chinese Diagnostic Criteria of Mental Disorders （CCMD-2-R, CCMD-3） and WAIS-R. Results： There were 69 patients （37.3 % ） met the criteria of intellectual impairment. The factors including self-care ability disorder, range of brain injury and positive signs of nervous system showed significant differences between the patients with and without intellectual impairment （ P 〈 0. 05）. By multiple regression of the related factor analysis, the self-care ability, positive signs of nervous system and the range of brain injury were entered in the regression equation in turn. Conclusion：The occurring of the intellectual impairment after brain injury in forensic assessment have relation with the factors of the self-care ability disorder, positive sign of nervous system and the range of brain injury.
