干燥综合征患者唾液pH值、电解质测定的诊断意义初步探讨

摘　　要：目的通过测定干燥综合征患者唾液pH值、Na+、K+浓度,计算比值及单位时间分泌量,探讨唾液生化指标对干燥综合征的诊断价值。方法分为干燥综合征病变组和正常对照组,吐取法采集混合唾液,测定唾液pH值、Na+、K+浓度, 计算Na+／K+,Na+、K+单位时间分泌总量。t检验统计分析。结果干燥综合征病变组唾液Na+、K+浓度、Na+／K+明显高于正常对照组,与正常对照组相比,病变组K+单位时间分泌总量明显下降,pH值显著降低。Na+浓度、Na+／K+敏感性、特异性均较高,pH值特异性较高,K+单位时间分泌总量敏感性、特异性一般。结论唾液检查是一种简便、准确、无创的检查手段,干燥综合征患者唾液pH值、电解质具有特征性,其综合评定对干燥综合征有一定诊断价值。Objective To define the reference values of several salivary variables, which had been proved to be potentially relevant to diagnosing Sj6gren syndrome （SS） in a previous study. The sodium,potassium concentration,sodium-potassium ratio,total value per minute of sodium and polassium and pH in patients with SS and normal contrls were studied in order to offer a possible new and non-invasive way of diagnosing SS. Methods The subjects were divided according to the European classification criteria for diagnosis of SS and normal controls. The concentration and total value of electrolytes and pH in mixed saliva were examined in both groups. Independent-sample t test were used to determine. Results The salivary concentration of sodium, potassium and sodium-potassium ratio were significantly elevated and the tolal value per minute of potassium and pH were significantly reduced in patients with SS. Suggesting that combing sodium concentration （high specificity 90% and high sensitivity 93.3%）and sodium-potassium ratio （high specificity 96.7% and high sensitivity 90%）and pH（high specificity 93.3% and moderate sensitivity 80%）as salivary variables could be used for diagnosing SS. Conclusion Because these non-in-vasive diagnostic tools can be easily applied,and are very accurate,so sialochemistry may be recommended as a diagnostic test for the diagnosing and differentiating Sjoegren syndrome from other rheumatic diseases.
