摘 要:目的观察烧灼兔睑板腺开口后干眼的形成与发展过程,探索新的干眼模型制作方法。方法健康新西兰大白兔16只,每只随机选择一眼,采用烧灼睑板腺开口的方法制作干眼模型,术后定期检查睑缘肿胀、睑缘充血、睑缘痂皮、睑板腺开口数目、角膜荧光素染色、虎红染色、羊齿状试验、SchirmerⅠ试验、结膜杯状细胞密度等的变化情况。结果术后第1天,实验眼眼睑肿胀,睑缘充血、大量黄白色痂皮,术后第7天至第9天眼睑反应明显减轻,睑板腺开口未见重新开放。术后第7天及第28天以后实验眼角膜荧光素染色、虎红染色积分较对照眼同一时间积分有明显升高(P<0.05),术后42 d实验眼羊齿状试验Ⅲ、Ⅳ级结晶增多(P=0.000),结膜杯状细胞密度下降(P=0.0087),术后45 d病理检查实验眼可见睑板腺导管轻度扩张。结论烧灼兔睑板腺开口是一种简单可行的兔干眼模型制作方法。Objective To evaluate the characteristic of rabbit' s dry eye model by cauterizing the meibomian gland orifices and search new rabbit' s dry eye model. Methods Sixteen New Zealand white rabbits were experimented. The meibomian gland orifices of one eye of every rabbit were cauterized, the other eye was used as control. Eyelid swelling, lid margin hyperemia, lid margin eschar, meibomian gland orifices, rose bengal stain, fluorescence stain, ferning test and Schirmer Ⅰ test ( after anesthesia ) were performed before operation and 1, 2, 4, 7, 14, 21, 28, 35, 42 days after operation( Schirmer Ⅰ test was not carried out at 1, 2 days after operating). Conjunctival impression cytology test was done at 14, 28 and 42 days before and after operation. At the 45th day, eyelid, conjunctiva, and cornea tissues biopsy was carried out under optical microscope. Results At the first day after operating, all cauterized eyes presented eyelid swelling, lid margin hyperemia, and eschar at the cautery site, which was reduced in 7 to 9 days. All meibomian gland orifices were closed all along. By the 4th week, abnormal rose bengal stain and fluorescence stain could be found in the experiment group ( P 〈 0.05 ). By the sixth week, Ⅲ,Ⅳ grade fern increased in the experiment group, and the conjunctival goblet cell density was lower than the control group. Under optical microscope examination, some meibomian gland ducts was dilated in the experiment group. Conclusion Rabbit dry eye model by cauterizing the meibomian gland orifices is simple and practical.
