摘　　要：目的：探讨立体定向放射治疗（SRT）肝癌伴门静脉癌栓、下腔静脉与右心房癌栓的临床疗效。方法：采用立体定向伽玛射线体部治疗系统治疗8例肝癌伴门静脉癌栓、下腔静脉与右心房癌栓患者。根据肿瘤与癌栓的位置、临床靶体积、病人的身体状况与治疗目的，制定放射治疗计划及调整剂量分布。等剂量曲线50％，分割处方剂量380-450cGy，重复治疗7-9次，隔日治疗。结果：治疗结束后1-3个月，生存质量明显改善。复查CT或MR，肿瘤治疗有效率为62．5％。癌栓得以缩小与控制。平均生存时间为11．6个月。结论：SRT治疗肝癌伴门静脉癌栓、下腔静脉与右心房癌栓，临床效果较为理想，为患者提供了一种有效的治疗方法．Objeotive：To research clinical therapeutic effectiveness of stereotactic radiotherapy（SRT） in hepatocellular carcinoma with tumor thrombi in the portal vein, inferior vena cava, and right cardiac atrrium, Methods： 8 patients with the hepatocellular carcinoma with tumor thrombi in the portal vein, inferior vena cava, and right cardiac atrrium were therapied with the Stereotactic R-ray Radiotherapy System. The radiotherapeutic planning and setting of dosage distribution had been worked out on the basis of the carcinoma and tumor thrombi location, clinical target volume, patient health condition, and therapeutic aim. The isodosage curve was 50%. The fractional doses was 380-450 cgy. The therapy had been repeated for 7-9 times, every two days one time. Results ： After the therapy 1 to 3 months. The survival quality of patients was improved significant. The patients received CT, or MR test after therapy. The therapeutic efficiency rate of carcinoma are 62.5%. The tumor thrombi shrank and controlled.The general median survival period were 11.6 months. Conclusion： It could give accurate and high dose irradiation with SRT to the tumor in hepatocellular carcinoma with tumor thrombi in the portal vein. There is only mild damage on surrounding tissue. The therapeutic effectiveness of SRT in hepatocellular carcinoma with tumor thrombi in the portal vein, inferior vena cava, and right cardiac atrrium is satisfactory. It offer a effective therapeutic method for the patient.
